活血消肿汤联合低分子量肝素钙对骨科大手术术后下肢深静脉血栓形成预防作用的临床观察

         

摘要

Objective To observe the clinical effects of blood-activating and detumescence recipe combined with low molecular weight heparin calium on the treatment of deep vein thrombosis of lower limbs after orthopaedics major operation.Methods 146 patients who were done orthopaedics operation in our hospital were randomly divided into two groups, the control group (n=75) was treated by simple subcutaneous injection of low molecular weight heparin calium, the treatment group (n=71) was treated by blood-activating and detumescence recipe on the basis of control group.The curative effects were evaluated after treatment 14 d in two groups.The circumference of limb thigh and calf,D-dimer prothrombin time, hemorheology, blood rheology and blood routine and double-limbs color doppler ultrasound before and after treatment were observed, for checking the deep vein to clear whether the formation of thrombosis.Results The circumference mean of ipsilateral limb thigh and calf after treatment in treatment group were lower than that in control group(P<0.05).The improvement of the whole blood viscosity and plasma viscosity after treatment in treatment group were superior to those in control group (P<0.05).There was no statistical difference on hematocrit value, D-dimer, prothrombin time and color doppler ultrasound after treatment between two groups (P>0.05).Conclusion Blood-activating and detumescence recipe combined with low molecular weight heparin calium can effectively prevent the formation of deep vein thrombosis of lower limbs after orthopaedics operation.%目的 观察活血消肿汤联合低分子量肝素钙预防骨科大手术术后下肢深静脉血栓形成的临床疗效.方法 将146例骨科大手术的患者随机分为2组,对照组75例予单纯皮下注射低分子量肝素钙治疗,治疗组71例在对照组治疗基础上加用活血消肿汤治疗.2组均治疗14 d统计临床疗效,并观察2组治疗前后大腿、小腿周径变化,D-二聚体、凝血酶原时间、血液流变学及血常规变化,以及彩超检查双下肢深静脉情况,明确有无深静脉血栓形成.结果 治疗组治疗后患侧肢体大腿、小腿周径均值均低对照组治疗后(P<0.05),全血高切、中切、低切黏度及血浆黏度改善均优于对照组(P<0.05),而2组治疗后红细胞比容、D-二聚体、凝血酶原时间及患侧深静脉彩超未见明显差异(P>0.05).结论 活血消肿汤联合低分子量肝素钙能有效预防骨科术后下肢深静脉血栓形成.
