陈旧性脑损害继发皮质脊髓束Wallerian变性的MRI表现

     

摘要

目的:分析陈旧性脑损害继发皮质脊髓束Wallerian变性的MRI表现,提高该病的诊断正确率。方法收集扬州市中医院2012年9月1日至2016年2月29日期间收治的陈旧性脑损害继发皮质脊髓束Wallerian患者27例,MRI扫描序列包括T1Flair、快速自旋回波序列(FSE) T2WI、T2Flair及DWI功能成像,并对影像资料进行分析并总结。结果在原发病变基础上,可见继发Wallerian变性均沿皮质脊髓束方向分布,呈点、条片状异常信号影,MRI表现为T1略低信号、T2WI及T2Flair略高信号,DWI呈等、高信号;27例伴有同侧大脑脚萎缩,6例伴有同侧脑桥萎缩。结论 MRI是诊断陈旧性脑损害继发皮质脊髓束Wallerian变性的可靠方法。%Objective To analyze the magnetic resonance imaging (MRI) findings of corticospinal tract Walle-rian degeneration secondary to old brain injury and to improve the diagnostic accuracy of the disease. Methods The im-aging data of 27 cases of corticospinal tract Wallerian degeneration secondary to old brain injury were collected and ana-lyzed. The scanning sequences of MRI included T1 Flair, fast spin echo (FSE) T2WI, T2 Flair and DWI. Results Sec-ondary Wallerian degeneration distributed along the direction of the corticospinal tract on the basis of the primary lesion, with abnormal signal shadow in the shape of points, strip, and plate. MRI performances included slightly low intensity on T1WI, slightly hyperintense sign on T2WI and T2 Flair, isointense and hyperintense sign on DWI. There were 27 pa-tients with insilateral cerebral peduncle atrophy and 6 patients with insilateral pons atrophy. Conclusion MRI is valu-able in the diagnosis of corticospinal tract Wallerian degeneration secondary to old brain injury.
