慢性心力衰竭患者心功能与血浆IL-18、IL-37及IL-33的相关性研究

         

摘要

目的 探讨慢性心力衰竭患者血浆细胞炎症因子 IL-18、IL-33及IL-37 蛋白水平变化与心功能的相互关系.方法 收集 120 例慢性心力衰竭患者,按 NYHA 心功能分级将患者分为三组:CHF1 组(NYHA Ⅱ级)、CHF2 组(NYHA Ⅲ级)、CHF3 组(NYHA Ⅳ级)各40 例,彩色超声诊断仪测定心功能,ELISA酶联免疫吸附试验法测定各组患者血浆 IL-18、IL-33 及 IL-37 蛋白水平.结果 三组患者血浆 IL-18、IL-33 及 IL-37 蛋白水平均随着NYHA心功能分级的升高而显著升高(P﹤0.01).CHF 患者血浆 IL-18 与 IL-33 蛋白水平呈正相关关系(r=0.718,P﹤0.01),CHF 患者血浆 IL-18 与 IL-37 蛋白水平呈正相关(r=0.583,P﹤0.01),CHF 患者血浆 IL-33 与IL-37 蛋白水平呈正相关(r=0.546,P﹤0.01).结论 炎症是慢性心力衰竭发生、发展中的重要影响因素,细胞炎症因子 IL-1 8、IL-3 3 及 IL-3 7 的水平监测为临床治疗慢性心力衰竭提供依据.%Objective Objective to investigate the relationship between the changes of plasma inflammatory cyto-kines IL-18,IL-33 and IL-37 protein levels and cardiac function in patients with chronic heart failure. Methods:Methods a total of 120 patients in Department of Cardiology,outpatient and outpatient departments in our hospital from December 2016 to December ~2017 were enrolled in this study. The patients were divided into 3 groups ac-cording to the NYHA cardiac function classification:The CHF1 group(NYHA Ⅱ),CHF2 group(NYHA Ⅲ),group CHF3(NYHA Ⅳ),each group of 40 cases,the determination of IL-18,IL-33 and IL-37 protein levels in plasma ELISA elisa. Results There was statistical significance between IL-18,IL-33 and cardiac function in patients with dif-ferent IL-37 grading(P﹤0.05);plasma of patients with chronic heart failure IL-18,IL-33 and IL-37 protein levels were increased with increasing of NYHA grade of heart function and the difference was statistically significant(P﹤0.01)CHF3(NYHA Ⅳ)group and CHF1(NYHA Ⅱ)group,IL-18,IL-33 and inflammatory factors of IL-37 were significantly increased,there was a significant difference(P﹤0.01). CHF2(NYHA Ⅲ)group and CHF1(NYHAⅡ)had significant difference(P﹤0.05)CHF plasma levels of IL-18 and IL-33 protein level was positively correla-ted(r=0.718,P﹤0.01),CHF plasma levels of IL-18 and IL-37 protein levels were positively correlated(r=0.583, P﹤0.01),CHF and IL-37 protein levels in plasma of patients with IL-33 positive(r=0.546,P﹤0.01).Conclusion Inflammation is an important factor in the occurrence and development of chronic heart failure. Monitoring of the levels of cell inflammatory factors IL-18,IL-33 and IL-37 provides a basis for clinical targeted treatment of chronic heart failure.
