改良皮瓣法腋臭切除术在并发症防治中的作用

         

摘要

目的 探讨改良皮瓣法腋臭切除术在并发症防治中的作用.方法 对腋臭76例采用改良皮瓣法腋臭切除术.手术去除大汗腺及毛囊后在皮瓣上用尖刀片沿皮纹戳数个长约3 mm的微孔进行引流,缝合切口后用弹力绷带包扎压迫手术区敷料.术后3d及7d换药,10 d拆线并观察皮瓣成活情况,术后3~6个月进行随访.结果 76例术后均无血肿及感染等并发症,切口均甲级愈合.随访腋部辛辣汗臭均消失、出汗减少,社交活动正常;其中5例腋窝瘢痕宽1~3 mm,余患者瘢痕不明显,皮肤颜色及皮肤质地均正常,腋毛基本消失.结论 带微孔引流改良皮瓣在保证腋臭手术效果的同时能减少手术并发症的发生.%Objective To explore the effect of improved skin flap in prevention and treatment of complications of axillary osmidrosis. Methods 76 patients with axillary osmidrosis underwent ectomy with improved skin flap. Several micropores were punctured with sharp knife along dermatoglyph after excision ol apocnne sweat glands and hair follicle. The nucropore was used for drainage, and its width was about 3 mm. The skin was sewed up and pressure dressing was applied on both axillas with elastic bandage. Dressings were changed on the 3rd day and 7th day respectively, and the sutures were removed on 10th day to observe the situation of skin flaps. All the patients were followed up for 3-6 months alter operation. Results All the incisions healed well, and no hematoma, infection or other complications occurred after operation. The incisions healing were of the first rate. Hot stink of sweat smell disappearance, sweating decrease and normal social activities were observed m the follow-up, except 5 eases with sears of 1 to 3 mm in size. The color and the texture of axillary skin were normal, and almost all the armpit hair vanished. Conclusion The improved skin flap with micropore as drainage can achieve better results in the treatment of axillary osmidrosis with fewer complications.
