乳腺癌组织中转录抑制因子增强子和p53蛋白表达与临床病理特征及预后的关系

         

摘要

目的 探讨乳腺癌组织中转录抑制因子增强子(EZH2)和p53蛋白的表达及其与临床病理特征及预后的关系. 方法 选择病理科存档的乳腺癌组织石蜡块107例为乳腺癌组,从中随机抽取50例患者取其癌旁正常乳腺组织作为癌旁组织组,采用免疫组化方法分析患者EZH2、p53蛋白表达情况,分层分析EZH2、p53蛋白表达与患者临床病例特征的关系,并采用生存曲线分析EZH2、p53蛋白表达与患者预后的相关性. 结果 EZH2和p53蛋白在乳腺癌组织中的表达阳性率均明显高于癌旁组织组,差异均有统计学意义(χ2分别=21.72、28.94, P均<0.05). 免疫组化染色显示,EZH2和p53蛋白在乳腺癌组织中可见明显的阳性表达. EZH2阳性表达与患者肿瘤直径、淋巴结转移、病理分级、雌激素受体相关(χ2分别=11.72、7.65、7.39、4.07,P均<0.05),p53蛋白阳性表达与患者肿瘤直径、TNM分期、病理分级相关(χ2分别=6.51、6.97、7.87,P均<0.05). Kaplan-Meier 生存曲线分析显示,EZH2蛋白表达与患者生存预后具有明显影响 (χ2=3.93, P<0.05), 而p53蛋白表达与患者生存预后无明显影响 (χ2=1.63,P>0.05). 结论 乳腺癌组织中EZH2、p53蛋白阳性表达率较正常乳腺组织增高,且EZH2、p53蛋白与患者肿瘤直径、临床分期、淋巴结转移等病理特征有关,其中EZH2蛋白与患者生存预后有关.%Objective To investigate the expressions of p53 and EZH2 in breast cancer and its relationship with clinical pathological features and prognosis. Methods A total of 107 samples of breast cancer paraffin blocks from pathology de-partment archived were enrolled as breast cancer group, and 50 cases out of 107 cases were randomly selected to obtain normal breast tissues adjacent to the carcinoma as the control group. The EZH2 and p53 protein expressions were analyzed by immunohistochemical method and their relationship to clinical characteristics were analyzed. The correlation between EZH2 and p53 protein expressions and the prognosis was analyzed by survival curve. Results The expression positive rates of EZH2 and p53 protein in breast cancer tissues were significantly higher than those in paracancerous tissues (χ2=21.72,28.94, P<0.05). Immumohistochemical staining showed that EZH2 and p53 protein were positive expressed in breast cancer tissue. EZH2 expression was associated with tumor size, lymph node metastasis, pathological grade and es-trogen receptor (ER)(χ2=11.72,7.65,7.39,4.07,P<0.05) and p53 protein expression was related with tumor size, TNM stage and pathological grade(χ2=6.51,6.97,7.87,P<0.05). EZH2 protein expression had obviously effect on survival prog-nosis(χ2=3.93,P<0.05) while the p53 protein expression showed no effect on survival prognosis(χ2=1.63, P>0.05). Con-clusion The expression positive rates of EZH2 and p53 protein in breast cancer tissues is higher than normal breast tis-sue, and EZH2 and p53 protein are related with tumor size, clinical staging, lymph node metastasis and other pathological characteristic. EZH2 protein is related to survival prognosis.
