弹性袜联合抗栓泵在预防妇科盆腔手术后下肢深静脉血栓形成护理中的应用

摘要

目的 探讨弹性袜联合抗栓泵在预防妇科盆腔手术后下肢深静脉血栓(DVT)形成护理中的应用价值.方法 选取2013年8月至2016年8月我院实施妇科盆腔手术患者160例为研究对象,采用随机数表法分为观察组和对照组各80例,对照组术后采取常规护理模式,观察组在此基础上采用弹性袜联合抗栓泵,均干预2个月,测定活化部分凝血活酶时间(aPTT)、凝血酶时间(TT)、纤维蛋白原(Fbg),并采用彩色多普勒超声检查仪检测双下肢股静脉血流速度,应用健康调查简表(SF-36)、视觉模拟评分(VAS)、护理满意度评分表分析两组健康状况、疼痛度及护理满意度,同时记录DVT形成率.结果 干预后观察组aPTT与对照组比较明显延长(P<0.05),观察组Fbg低于对照组(P<0.05);干预后观察组左股静脉流速、右股静脉流速与对照组比较明显加快(P<0.05);观察组SF-36评分、满意度评分高于对照组(P<0.05),VAS评分分较对照组低(P<0.05);观察组DVT形成率2.50%与对照组11.25%比较明显较低(P<0.05).结论 弹性袜联合抗栓泵可有效改善妇科盆腔手术患者凝血功能及双下肢股静脉血流速度,预防下肢深静脉血栓形成,降低疼痛并提高生活质量.%Objective To investigate the value of elastic stockings combined with antithrombotic pump in the prevention of lower extremity deep venous thrombosis ( DVT ) after gynecological pelvic operation . Methods A total of 160 patients who underwent gynecological pelvic operation in the hospital between August 2013 and August 2016 were selected as study subjects and were divided into the observation group and the control group ,80 cases in each group .After operation ,the control group received routine nursing ,and the observation group were additionally treated with elastic stockings combined with antithrombotic pump .Both groups received 2 months of intervention .The activated partial thromboplastin time ( aPTT ) , thrombin time (TT)and fibrinogen(Fbg)were determined.The femoral venous blood flow velocity of both lower extremities was detected by color Doppler ultrasound .The health status ,pain degree and nursing satisfaction were analyzed with health status Short Form ( SF-36 ) , visual analogue scale ( VAS ) and nursing satisfaction scale , and the incidence of lower extremity DVT was recorded .Results After the intervention , aPTT and TT of the observation group was significantly longer than those of the control group ( P<0.05 ) ,and Fbg level was lower than that in the control group(P<0.05).After the intervention,the left and right femoral venous blood flow velocity of the observation groupwas faster than the control group (P<0.05).The SF-36 score and satisfaction score of the observation group were higher than those of the control group (P<0.05),while the VAS score was lower than that of the control group ( P<0.05) .The incidence of DVT in the observation group (2.50%) was lower than that in the control group ( 11.25%) ( P <0.05 ) .Conclusion Elastic stockings combined with antithrombotic pump can effectively improve the blood coagulation function and femoral venous blood flow velocity of both lower extremities ,prevent lower extremity deep vein thrombosis ,reduce the pain and improve the quality of life of patients undergoing gynecological pelvic operation .
