Objective To compare the effects between aspirin and low molecular weight heparin (LMWH) in preventing lower extremity deep venous thrombosis (DVT)after surgery of great saphenous varicose veins.Methods 92 patients with great saphenous varicose veins who underwent surgery in our hospital between July 2014 and July 2016 were divided into two groups according to different treatment methods,46 cases in each group.Patients treated by oral administration of aspirin before the surgery (aspirin group),while patients treated by injection of LMWH before and after the surgery (LMWH group).The incidence rates of postoperative DVT,duration of surgery,total blood loss,decreased hemoglobin levels,postoperative prothrombin time (PT),activated partial thromboplastin time (aPTT),thrombin time (TT) and other blood coagulation indexes and the incidence rates of other postoperative complications were compared between the two groups.Results The incidence rate of postoperative DVT in aspirin group (41.30％)was higher than LMWH group (17.39％) (P ＜ 0.05);The levels of PT,aPTT,TT and other blood coagulation indexes in the LMWH group were higher than aspirin group after surgery (P ＜ 0.05);The incidence of other postoperative complications in aspirin group (44.83 ％)was higher than tLMWH group (10.87 ％) (P ＜ 0.05).Conclusion LMWH plays a positive role in prevention of DVT after surgery of great saphenous varicose veins,and the effect is better than aspirin.The former can improve the coagulation state,and will not increase the risk of bleeding.%目的 研究比较阿司匹林与低相对分子质量肝素(LMWH)预防大隐静脉曲张手术后下肢深静脉血栓(DVT)形成的效果.方法 选取我院2014年7月至2016年7月行大隐静脉曲张手术患者92例,根据治疗方法的不同将所有患者分为两组,每组46例,一组患者在行大隐静脉曲张手术前后服用阿司匹林(阿司匹林组),另一组患者在手术前后注射LMWH(LMWH组),比较两组患者术后DVT发生率,比较两组患者手术时间、总失血量、血红蛋白下降水平以及术后凝血酶原时间(PT)、活化部分凝血酶时间(aPTT)、凝血酶时间(TT)等凝血指标水平,比较两组患者术后其他并发症发生率.结果 阿司匹林组患者行大隐静脉曲张手术后DVT发生率为41.30％较LMWH组17.39％高(P＜0.05);LMWH组患者术后PT、aPTT、TT等凝血指标水平较阿司匹林组高(P＜0.05);阿司匹林组患者术后其他并发症发生率为44.83％较LMWH组10.87％高(P＜0.05).结论 LMWH对大隐静脉曲张手术术后DVT发生有着积极的预防作用,预防效果显著优于阿司匹林,患者的凝血状态得到改善,且不会增加出血风险.
