伴有变应性鼻炎小儿鼾症序列治疗

Objective To explore the effective method for the treatment of allergic rhinitis in children with sleep apnea. Methods Case is in May 2015 to June 2016 of 60 patients admitted in our hospital outpatient with severe allergic rhinitis children with sleep apnea, based on the principles of randomized divided into experimental group (30 cases) and control group (30 cases), all cases to use antibiotic treatment for 1 week, at the same time, combination of Fluticasone Propionate Nasal Spray, Levocetirizine Dihydrochloride Tablets treatment for 1 week; The experimental group since morning on an empty stomach with 5 mg Prednisone, more than 9 years old children's oral 10 mg, discontinuation after 1 week in a row, After the meal with a piece of Calcium Supplement with Vitamin D Chewable Tablets, Control group given a placebo. A week after the two groups at the same time, combination of Fluticasone Propionate Nasal Spray, Levocetirizine Dihydrochloride Tablets, Montelukast Sodium Tablets and Vitamin D Chewable Tablets treatment of 4 weeks. Before and after 1 week treatment, 4 weeks after treatment the patient's symptom scores according to Carlos O' Connor-Reina parents questionnaire. Results Compared with before treatment, the experimental group and control group symptom scores were significantly decreased trend, the difference had statistical significance (P<0.05).1 week after treatment, the experimental group the symptom scores were lower than the control group, the differences were statistically significant (P<0.05); Experimental group total effectiveness 100%, control group total effective rate was 40%, two groups of efficient difference was statistically significant (P<0.05). 4 weeks after treatment, the experimental group each symptom scores were lower than the control group, the differences were statistically significant (P<0.05);Experimental group total effectiveness 100%, control group total effective rate 63.33%, two groups of efficient difference was statistically significant (P<0.05). Conclusion Prednisone combined with vitamin D calcium chewable tablets in severe allergic rhinitis in children with sleep apnea treatment had obvious auxiliary therapeutic effect.%目的 探讨伴有严重过敏性鼻炎小儿鼾症的有效治疗方法.方法 病例为2015年5月至2016年6月我院门诊收治的60例伴有严重过敏性鼻炎小儿鼾症,依据随机对照原则分为实验组(30例)与对照组(30例),所有病例使用抗生素治疗1周,同时联合使用丙酸氟替卡松鼻喷雾剂、左西替利嗪治疗1周;实验组晨起空腹加用强的松5mg,超过9岁口服10mg/d,连续用药1周后停药,饭后再加用一片儿童维生素D钙咀嚼片,对照组同期给予安慰剂.一周后两组同时联合使用丙酸氟替卡松鼻喷雾剂、左西替利嗪、孟鲁司特钠以及维生素D钙咀嚼片治疗4周;治疗前、治疗1周后、治疗4周后患者的症状评分根据Carlos O'Connor-Reina的家长调查问卷获得.结果 与治疗前相比,实验组及对照组症状评分均呈明显下降趋势,差异均有统计学意义(P<0.05).治疗后第1周,实验组各项症状评分均低于对照组,差异均有统计学意义(P<0.05);实验组总有效率100%,对照组总有效率40%,二组有效率差异有统计学意义(P<0.05).治疗后第4周,实验组各项症状评分均低于对照组,差异均有统计学意义(P<0.05);实验组总有效率100%,对照组总有效率63.33%,二组有效率差异有统计学意义(P<0.05).结论 强的松联合维生素D钙咀嚼片在伴有严重过敏性鼻炎小儿鼾症治疗中有明显的辅助治疗作用.
