老年综合评估与老年失代偿期心力衰竭住院患者死亡率的关系研究

摘要

目的:探讨老年综合评估(CGA)与失代偿期心力衰竭老年住院患者死亡率的关系。方法选取2013年7月—2015年7月在老年医学科住院、≥75岁的失代偿期心力衰竭患者,共134例为研究对象,展开 CGA 评分,应用 Logistic 回归分析 CGA 评分对失代偿期心力衰竭老年住院患者死亡率的影响。结果死亡患者与存活患者在年龄、血脂、房扑/房颤、平均血压及心功能分级等方面差异均有统计学意义(P <0.05)。CGA 评分体系中,死亡患者与存活患者在健康评价、日常生活能力和精神状况等方面差异均有统计学意义(P <0.05)。死亡患者与存活患者 CGA 评分在评分构成上差异均有统计学意义(P <0.001)。Logistic 回归筛选危险因素:年龄、血脂、心功能分级及 CGA 评分为失代偿期心力衰竭老年住院患者死亡的危险因素,共同影响其预后,P 值(双侧)均<0.05。结论对于失代偿期心力衰竭老年住院患者,年龄、血脂、心功能分级及 CGA 评分是其住院死亡率的危险因素。%Objective To assess the relationship between the comprehensive geriatric assessment (CGA)score and the mortality rate of elder inpatients with decompensated chronic heart failure (CHF). Methods One hundred and thirty-four elder inpatients who were more than 75 years and diagnosed with decompensated CHF in Department of Geriatrics from July 201 3 to July 201 5 were included and assessed by CGA.Logistic regression analysis was used to evaluate the effects of CGA scores on the mortality rate of elderly inpatients with decompensated CHF.Results Significant differences between death patients and survival patients were found in age,serum lipid,atrial flutter/atrial fibrillation,mean blood pressure and cardiac function classification (P <0.05).In the CGA scoring system,significant differences between death patients and survival patients were found in health assessment,ability of daily life and mental state (P <0.05).The significant difference of the CGA scores between death patients and survival patients appeared in the score constitution(P <0.001 ).In Logistic regression analysis,age,lipid level,cardiac function classification,CGA score were screened as the risk factors of the death in the decompensated CHF inpatients,which together influenced the prognosis of inpatients,so the value of P(bilateral)<0.05.Conclusions In the study of elder inpatients with decompensated CHF,age,serum lipid,cardiac function classification and the CGA score are the risk factors of the mortality rate of inpatients with decompensated CHF.
