全肺灌洗治疗肺泡蛋白沉积症的疗效观察

摘要

目的：通过对采用全肺灌洗（whole-lung lavage，WLL）治疗的肺泡蛋白沉积症（pulmonary alveolar proteinosis，PAP）患者的病例资料的总结和分析，评价住院和随访期间的疗效。方法回顾性分析2010年1月至2015年5月本院确诊PAP的12例患者在给予WLL治疗前后肺功能、血气分析的变化，并随访患者12个月死亡率和再住院率。结果12例PAP患者经过WLL治疗后1年内死亡率0%，再住院率25%；经过全肺灌洗治疗后用力肺活量、一秒用力呼气容积、一氧化碳的弥散量、血氧分压、氧饱和度较术前升高（P＜0.05）。结论 WLL治疗能改善患者肺功能和生活质量，减少1年期的再住院率和死亡率。%Objective To evaluate the efficacy of whole lung lavage in patients with pulmonary alveolar proteinosis by retrospective analysis.Method All of 12 patient were diagnosed as pneumonia and treated by whole-lung lavage (WLL) in our hospital from January 2010 to May 2015. Changes of lung function and arterial blood gas were analyzed after treatment, mortality and readmission rate in next 1 year were retrospectively analyzed.Result Mortality of cases was 0% and readmission rate was 25% in next 1 year. Patients’ forced vital capacity, Forced expiratory volume in one second, carbon monoxide diffusing capacity, partial pressure of oxygen, oxygen saturation increased (P<0.05) after WLL.Conclusion WLL therapy is likely to improve lung function and life quality of patients with PAP, and also reduce readmission rate and mortality in next 1 year.
