甘露聚糖肽胶囊在慢性阻塞性肺疾病稳定期的临床效果研究

摘要

Objective:To observe the clinical efficacy of mannatid capsules combined with seretide in treatment of chronic obstructive pulmonary disease ( COPD ) patients.Methods:366 patients with COPD were divided into observation group (n=183)and control group(n=183) randomly. The patients in observation group were administrated with mannatide capsules and seretide,those in control group were administrated with broncho-vaxom and seretide.In order to compare the indexes as follows:quality of life,exercise tolerance of patients,arterial blood-gas,lung function testing,IgA,C3,lymphocyte cells、NK cells、CD4+,CD8+,CD4+/CD8+,the times of acute exacerbation of chronic obstructive pulmonary disease (AECOPD),duration of every AECOPD and antibiotic consumption. Results:After treatment, EQ-VAS,the score of EQ-5D,exercise tolerance、lung function of two groups were increased,there were no significant differences between two groups (P<0.05). PaO2,PaCO2 of patients in observation group were more markedly ameliorated than those in the control group. Patients of two groups had less frequency and days of AECOPD, less antibiotic consumption without significant discrepancy. Indicators of CD4+,CD4+/CD8+,LYM,IgA,NK of the two groups were significantly ameliorated , CD8+ were significantly decreased before and after treatment(P<0.05).There were significant differences between two groups in terms of CD4+/CD8+,C3 (P<0.05). Conclution: Mannatide capsules can improve the immune function in patients with COPD, reduce the attack of AECOPD, improve the patient's clinical symptoms.%　　目的：观察甘露聚糖肽胶囊联合舒利迭治疗慢性阻塞性肺疾病(COPD)的疗效.方法：将366例慢性阻塞性肺疾病住院患者随机平均分为观察组和对照组.观察组给予甘露聚糖肽胶囊和舒利迭,对照组给予泛福舒和舒利迭,治疗后对患者的生活质量、运动耐力进行评分和分级,进行桡动脉血气分析和肺功能检测,测定免疫指标IgA、补体(C3)、淋巴细胞、NK细胞、CD4+、CD8+、CD4+/CD8+(比值)；观察12个月内AECOPD的发作次数、每次AECOPD发作持续时间及抗生素使用天数.结果：两组患者在治疗后EQ-VAS和EQ-5D评分、运动耐力、肺功能指标FEV1/FVC(%)、FEV1、FEV1%较治疗前升高(P<0.05),观察组较对照组升高更明显(P<0.05)；两组患者在治疗后PaO2、PaCO2较治疗前均改善,且两组间差别有显著性意义；治疗后两组AECOPD发作次数、发作持续天数、抗生素使用天数均减少(P>0.05),但差异无显著性意义；治疗后,两组CD4+、CD4+/CD8+、LYM、IgA、NK细胞含量等均较治疗前升高(P<0.05),CD8+较治疗前降低(P<0.05)；治疗组与对照组相比,CD4+/CD8+、C3含量升高更明显(P<0.05),而两组间CD4+、IgA、NK及CD8+无显著性差异.结论：甘露聚糖肽胶囊可全面提高COPD患者免疫功能,减少AECOPD的发作,改善患者的临床症状.
