股骨粗隆间骨折患者行内固定手术后的髋关节骨密度分析

摘要

Objective: To investigate the changes of the bone mineral density of the hip in elderly patients with intertrochanteric fractures after PFNAII internal fixation.Methods:100 cases with intertrochanteric fractures after PFNAII internal fixation were selected in this study.The bone mineral density of the healthy and the fractured hip was detected by the dual-energy X-ray(DXA)absorbtiometry at the 3 day and 90 day after the operation.Results:Contralateral hip femoral neck,greater trochanter of the femur intertrochanteric region,area and total hip bone mineral density value of district difference at the 90 days has not different with that in the 3 day after operation(P > 0.05),the bone density in the Ward's triangle area was significantly increased(P < 0.05).The 4 district area and the G area of the bone density of the patients at the 90 days after operation has little change with that in the 3day after operation(P > 0.05).The(1-9)district of the bone density of the patients significantly lower than that in the 3 day after the operation(P < 0.05).Conclusion:PFNAII internal fixation treatment can significantly increase the Ward's triangle bone mineral density of the health side of the patients,decreased bone mineral density of the (1-9)district.%目的:探讨老年股骨粗隆间骨折患者实施PFNAII内固定手术后髋关节骨密度的变化情况.方法:选取在我院实施PFNAII内固定手术治疗的股骨粗隆间骨折的老年患者100例,应用双能X线吸光测定法(DXA)与手术后3d以手术后90d分别测定患者健侧和患侧髋关节骨密度.结果:手术后90d较手术后3d,患者健侧髋关节股骨颈区、股骨大粗隆区、股骨粗隆间区以及全髋部区的骨密度值差别不大(P>0.05),Ward's三角区的骨密度值显著升高(P<0.05);患者手术后90d患侧3区、4区以及G区骨密度值与手术后3d相比变化不大(P>0.05),患者手术后90d患侧(1-9)区的骨密度值明显低于手术后3d的骨密度值(P<0.05).结论:PFNAII内固定治疗可明显升高患者健侧Ward's三角区的骨密度值,降低患侧(1-9)区的骨密度值.
