宫腔镜子宫内膜微创术对子宫内膜容受性因子表达的影响

摘要

Objective: To observe the hysteroscopic endometrial minimally invasive preoperative,postoperative factors related to endometrial receptivity expression and explore its clinical value for the embryo implantation rate and pregnancy rate. Methods:41 cases of infertility patients with continuous failure of in vitro fertilization and embryo transfer were selected as the research object.To understand the endometrial morphology,and the implementation of minimally invasive surgery.Detection of pre operative,postoperative uterine receptivity related factors ER,PR,LIF,VEGF expression.Results:After treatment with FSH,LH,E2,P and P/E2 values were higher than those before treatment,the statistical treatment,the difference was not statistically significant (t=1.751,0.075,1.109,1.239,P>0.05).After treatment with progesterone levels was significantly higher than that of treatment before and after statistical treatment,the difference is statistically significant (t=6.697,P<0.01);after the treatment of Er,PR,LIF and VEGF were expressed significantly higher than before treatment,statistically,the difference is statistically significant (t=22.9 09,86.388,8.880,28.415,P<0.01).Conclusion:Hysteroscopy endometrial minimally invasive surgery can enhance the expression of endometrial receptivity related factors ER,PR,LIF,VEGF,improve the endometrial receptivity,and then improve the implantation rate and clinical pregnancy rate.%目的：观察宫腔镜子宫内膜微创术术前、术后子宫内膜容受性相关因子表达情况，探讨其对胚胎植入率和妊娠率的临床价值。方法：选取体外受精-胚胎移植连续失败3次及以上的不孕患者41例为研究对象，宫腔镜下了解子宫内膜形态，并实施子宫内膜微创手术，检测术前、术后子宫内膜容受性相关因子子宫内膜雌激素受体（estrogen receptor，ER）、孕激素受体（progesterone receptor，PR）、白血病抑制因子（leukaemia inhibitory factor，LIF）、血管内皮生长因子（vascular endothelial growth factor，VEGF）的表达情况。结果：治疗后FSH、LH、E2、P、P/E2值均高于治疗前，经统计学处理，差异无统计学意义（t=1.751、0.075、1.109、1.239，P＞0.05），治疗后孕激素水平显著高于治疗前，经统计学处理，差异有高度显著性意义（t=6.697，P＜0.01）；治疗后ER、PR、LIF、VEGF术后表达显著高于治疗前，经统计学处理，差异有高度显著性意义（t=22.909、86.388、8.880、28.415，P＜0.01）。结论：宫腔镜子宫内膜微创术可以增强子宫内膜容受性相关因子ER、PR、LIF、VEGF的表达，改善子宫内膜容受性，有助于提高胚胎植入率和临床妊娠率。
