Objective To evaluate the clinical value of three-dimensional color Doppler flow imaging ( 3D-CDFI ) in the renal cell carcinoma with vascular characteristics. Methods The 3D-CDFI was used to analyze the interior and peripheral vascular characteristics of the tumors in 71 patients with renal cell carcinoma confirmed by surgery and pathological test. Results The accuracy rate of diagnosis by ultra-sonography was 95. 8％ ( 68/71 ). The vascular characteristics of renal cell carcinoma in 68 patients could be classified into 6 types, according to the different tapes, shape and amount of the vessels. Type Ⅰ , those with a few blood vessels ( 7 cases ); Type Ⅱ , those with poor blood vessels ( 8 cases ); Type Ⅲ , those with semicircular blood vessels (11 cases ); Type Ⅳ, those with circularity blood vessels ( 4 cases ); Type Ⅴ , those with rich blood vessels ( 26 cases ); Type Ⅵ, those with plentiful blood vessels (12 cases ). Conclusion The 3D-CDFI can intuitively and completely show vascular distribution and spatial structure in different pathological types of renal cell carcinoma because of its fair sensitivity, which can also provide a reliable method and play an important role in clinical diagnosis.%目的 探讨三维彩色多普勒超声成像(3D-CDFI)在肾癌血流分型中的应用价值.方法 术前采用3D-CDFI成像技术诊断71例肾癌患者,对肿瘤周边及其内部血流特点进行分析,并与术中所见及病理诊断结果对照.结果 71例肾癌患者中,超声诊断正确68例,符合率95.8%.根据肿瘤周边和内部血流信号的多寡、形态、走行的不同,将68例肾癌的血流分布表现归纳为6种类型:Ⅰ型,星点型(7例);Ⅱ型,少血流型(8例);Ⅲ型,抱球型(11例);Ⅳ型,环球型(4例);Ⅴ型,多血流型(26例);Ⅵ型,丰富血流型(12例).结论 3D-CDFI能较直观完整地显示不同病理类型的肾癌血流分布和空间结构,而且连续性较好,从而为临床诊断提供更多的信息,使诊断结果更加可靠.
