彩色多普勒超声观察输尿管喷尿流体动力学:与肾动态显像判定肾功能对比分析

         

摘要

Objective To explore the value of color Doppler ultrasound in observing ureter jet for testing renal function compared with renography. Methods Forty healthy kidneys and 70 kidneys with renal function impairment were enrolled. The patients drunk 500 ml water, and underwent color Doppler ultrasonography after 45 minutes for observing of bilateral urine jet. The frequency, peak velocity and duration time were measured to determine the extent of renal impairment by comparison with renogram. Results Renography showed that healthy kidneys could be seen 1 min after injecting contrast, and 2—4 min later, the images were clear and the radionuclide was uniformly distributed, while 3—5 min later, the kidney imaging gradually faded. Color Doppler ultrasound showed that the urine flow color was bright and the ureter jet was 4 to 10 times per minute, the duration was 5. 12—12. 38 s for each time, and the urinary peak velocity was 22. 36—79. 68 cm/s for normal kidneys. For kidney with impaired function, renography showed renal shadow with a delay washout, and radionuclide distributed roughly. Color Doppler ultrasound showed that the ureter jet became shorten, the color became dim, the frequency reduced in the affected side, or the ureters showed no urinary in severe cases. Conclusion By observing the ureter jet, color Doppler ultrasonography can determine whether the renal function is impaired as well as the extent of damage.%目的 与肾动态显像对比,评价彩色多普勒超声通过观察输尿管开口喷尿情况判定肾功能的价值.方法 选择40个正常肾脏和病变肾脏70个,检查前嘱患者饮水500 ml,45 min后用彩色多普勒观察喷尿时双侧输尿管尿流的频率、峰值速度和喷尿持续时间,判定肾脏损伤程度,并与肾动态显像进行对比.结果 正常肾脏:肾动态显像,注射显影剂1 min后双肾显影,2~4 min影像完整清晰,核素分布均匀,3~5 min后肾影逐渐变淡;彩色多普勒超声示喷尿时输尿管内尿流颜色鲜艳,每分钟喷尿4~10次,每次持续时间5.12~12.38 s,尿流峰值流速22.36~79.68 cm/s.肾功能受损时,肾动态显像肾影出现和消退延迟,肾影内核素分布稀疏;彩色多普勒超声示患侧尿流射程缩短,颜色暗淡,喷尿频率减低,严重者无尿流显示.结论 通过彩色多普勒超声观察输尿管喷尿情况,能够判定肾功能有无受损及受损程度.
