HIV感染者不同亚群T细胞IL-21及IL-21受体表达研究

         

摘要

目的:对HIV感染者不同亚群T细胞IL-21分泌及IL-21受体表达进行研究,并探讨其与疾病进展的关系.方法:选取34名HIV感染者及11名健康对照,应用流式细胞仪胞内染色技术在单细胞水平检测PMA/离子霉素刺激下CD4+、CD8+T细胞IL-21分泌及IL-21受体表达水平,分析其与病毒载量、CD4+T细胞及CD8+T细胞的相关性.结果:58.82%及54.71%HIV感染者检测到CD4+IL-21+T细胞(1.44±1.73)%及CD8+IL-21+T细胞(1.39±1.78)%,41.18%及45.29%的HIV感染者未检测到CD4+IL-21+T细胞及CD8+IL-21+T细胞;HIV感染者中,病毒载量低于20 000拷贝/ml组CD4+IL-21+T细胞(1.27±1.55)%显著高于病毒载量高于20 000拷贝/ml组(0.55±1.45)%(P<0.05);检测到CD4+/CD8+IL-21+T细胞组的CD8+T细胞数量[(951.07±309.24)、(966.49±292.91)个/μl]显著低于未检测到CD4+/CD8+IL-21+T细胞组[(1 258.48±338.30)、(1 282.11±376.28)个/μl](P<0.05);HIV感染者CD8+IL-21+T细胞与CD8+T细胞绝对值呈显著负相关(P<0.05).HIV感染者中,CD4+T细胞高组(> 350个/μl)CD4+、CD8+T细胞上IL-21R表达百分比[(2.52±1.53)%、(2.70±1.77)%]显著高于CD4+T细胞低组[<350个/μl,(1.49±3.27)%、(1.43±2.47)%,P=0.015;P=0.005].结论:HIV感染者CD4+IL-21+T细胞及IL-21受体高表达与低病毒载量及高水平CD4+T细胞相关,可能对疾病进展具有保护作用.%Objective:To detect the alternation of the level of IL-21 and IL-21 receptor expression on different subsets of T cells in HIV infection, and to study the relationship between IL-21 and IL-21 receptor expression on different subsets of T cells and disease progression of HIV infection. Methods:34 untreated HIV infected patients and 11 normal controls were enrolled and the level of CD4 +IL-21 + T cells ,CD8+IL-21+T cells, CD4+IL-21R+T cells and CD8+IL-21R+T cells were detected by flow cytometry. To study the correlation between the level of IL-21 and IL-21 R expression and disease progression, the absolute CD4 + T cells, CD8 + T cells and viral load were also examined. Results: We found that CD4 + IL-21 + T (1. 44 ± 1. 73 ) % and CD8 + IL-21 + T (1. 39 ± 1. 78)% could be detected in 58. 82% and 54. 71% of the HIV infected patients, respectively. While 41. 18% and 45. 29% of the HIV infected patients were with undetectable CD4+ IL-21 + T and CD8+ IL-21 + T cells. The level of CD4+ IL-21 + T cells was significantly higher in patients with relatively low plasma VL ( < 20 000 copies/ml) ( 1. 27 ± 1. 55 ) % compared with those with high VL > 20 000 copies/ml (0. 55 ± 1. 45) % (P < 0. 05). The absolute numbers of CD8 + T cells in HIV infected patients with detectable CD4 + IL-21 + T cells and CD8 + IL-21 + T cells [ (951. 07 ± 309. 24) , ( 966. 49 ± 292. 91 ) cells/u.1, respectively ] were significantly lower than that in patients with undetectable CD4+ IL-21 4 T cells and CD8 + IL-21 + T cells [ (1 258. 48 ± 338. 30 ), (1 282. 11 ± 376. 28) cells/julI , respectively]. The frequency of CD8 + IL-21 + T cells was inversely related to CD8 + T cells ( P < 0. 05 ). The HIV infected patients with relatively high CD4 + T cells ( > 350 cells/p,l) had higher level of CD4 + IL-21 R + and CD8 + IL-21 R + T cells compared with those with low CD4 + T cells ( <350 cells/ui) (P <0. 05). Conclusion:The high level of CD4+ IL-21 + T and IL-21R+T cells in HIV infected patients were correlated with relative low viral load and high CD4 + T cells, which suggested their roles in the protection of HIV disease progression.
