血清白蛋白浓度与缺血性卒中TOAST亚型预后的关系

Objective To investigate the association of serum albumin levels with outcome in ischemic stroke and its subtypes according to the TOAST criteria. Methods The study involved 394 consecutive stroke inpatients in our department. All included patients were classified into 5 major ischemic stroke subtypes based on TOAST criteria. Serum albumin levels were estimated in the 24 hours and follow-up interviews were conducted at 3 month postevent to determine stroke outcome. The association between serum albumin levels and stroke outcome was evaluated by t-test. Results There was lower levels of albumin at admission associated significantly with poor outcome (score of>3 on the modified Rankin Scale) than the good outcome (score of≤3 on the modified Rankin Scale). Differences were statistically significant. The reduced level of serum albumin associated significantly with poor outcome in all the stroke subtypes classified according to TOAST (Trial of ORG 10172 in Acute Stroke Treatment). Conclusion The increased serum albumin concentration can improve the prognosis of ischemic stroke.%目的：在TOAST分型的基础上，研究血清白蛋白浓度与缺血性脑卒中及其亚型预后之间的关系。方法连续性登记394例在神经内科住院治疗的急性缺血性卒中患者，依据TOAST分型标准确定每个患者所属病因学亚型，患者入院24 h内行血清白蛋白浓度测定，并随访3个月，确定卒中预后，通过t检验分析，研究血清白蛋白浓度与各TOAST亚型预后之间的关系。结果卒中预后较差（Rankin量表评分＞3分）的患者入院时血清白蛋白浓度低于预后较好（Rankin量表评分≤3分）的患者，差异有统计学意义。血清白蛋白浓度与各TOAST亚型预后皆有相关性。结论提高血清白蛋白浓度可以改善缺血性卒中预后。
