湖北省襄阳市男男性行为人群艾滋病干预效果分析

摘要

Objective To evaluate intervention effect HIV/AIDS related knowledge and behaviors among men who have sex with men (MSM),and provide evidence for controlling the spread of HIV among MSM.Methods MSM were recruited by using a classification of snowball method from 2012 to 2015,information of knowledge and behavioral were collected during the face-to-face interview.Blood samples were collected to detect HIV,syphilis and hepatitis C virus (HCV) antibody.All data were analyzed by the descriptive statistics methods and Analysis of Variance or trend Chi-square test.Results Through the integrated intervention among MSM,the correct awareness rate of all knowledge about H1V/AIDS were increased from 87.25％ to 92.50％,the accepting rate of propaganda,condom distribution or consulting increased from 89.50％ to 96.75％.The rate of consistent use of condom was increased from 30.50％ to 39.00％ and the rate of never using condom was decrease from 4.25％ to 0.75％ in the last six months.The rate of syphilis infection was decrease from 4.75％ to 1.50％,all tendency of them were significantly.But the rate of consistent use of condom was still lower than 40％,the separation phenomenon of knowledge and behavior was existed.The rates of HIV infection from 2012 to 2015 were 3.25％,4.00％,5.00％,6.75％,the tendency was significantly.The incidence of new infections increased,there was no statistical significance yet.Conclusion The effect was significant through the integrated intervention on MSM in Xiangyang.But the situation is still severe,so further effective and targeted measures are urgently to control of HIV among them.%目的 评价襄阳市男男性行为人群(MSM)艾滋病干预效果,为制定防控措施提供依据.方法 2012-2015年定期采用分类滚雪球法招募400名MSM,开展知识行为学调查和艾滋病病毒(HIV)、梅毒和丙型肝炎病毒(HCV)抗体检测.采用描述统计方差分析、趋势χ2检验分析方法比较各年度数据,分析干预效果.结果 通过综合干预,该市MSM艾滋病防治知识全部正确的比例由87.25％上升至92.50％,接受宣传、安全套发放或咨询的比例由89.50％上升至96.75％,最近6个月同性行为坚持使用安全套比例由30.50％上升至39.00％,同性行为从未使用安全套比例由4.25％下降至0.75％,梅毒阳性率由4.75％下降至1.50％,差异均有统计学意义.但安全套坚持使用率低于40％,存在知识和行为的分离现象.2012-2015年HIV抗体阳性率为3.25％、4.00％、5.00％、6.75％,上升趋势显著.新发感染率上升,但尚无统计学差异.结论 襄阳市MSM干预措施已取得一定成效,但防治形势依然严峻,应切实采取更加有效和针对性的措施,及时遏制艾滋病在该人群中蔓延.
