胃镜与胃电图在儿童腹型过敏性紫癜中的诊断价值

摘要

目的 分析儿童腹型过敏性紫癜的胃镜及胃电图特点,为腹型过敏性紫癜的早期诊断提供依据.方法 对我院儿科收治29例以腹痛为首发症状的过敏性紫癜患儿进行胃镜及胃电图检查,观察胃、十二指肠黏膜病变特点及胃电图的变化.结果 胃镜检查显示胃和十二指肠黏膜呈现不同程度的充血、水肿、糜烂甚至溃疡,以十二指肠降部病变为著,食管黏膜未见异常.腹型过敏性紫癜患儿胃电图示餐前、餐后平均幅值(Vpp)增高,主频率降低,餐前正常慢波百分比减少.结论 腹型过敏性紫癜的胃镜及胃电图具有特征性变化,为腹型过敏性紫癜的早期诊断提供依据.%Objective To analyze the characteristics of gastroscope and electrogastrogram (EGG) in the anaphylactoid purpura abdominalis of children, and offer a guide for early diagnosis. Methods Twenty-nine cases diagnosed anaphylactoid purpura abdominalis were examined by gastroscope and EGG. Mucosal lesions of stomach and duodenum were observed. Characteristics of EGG were analyzed. Results The gastroscopic characteristics of stomach and duodenal mucous were congestive, swelling, dissipated and even ulcer. Mucosal lesions became more serious in the descending duodenum. The esophageal mucous was normal. Compared with healthy children, preprandial and postprandial values of Vpp increased and that of dominant frequency decreased. Preprandial percent of normal slow wave decreased. Conclusion Gastroscope and EGG offer early diagnostic value in the anaphylactoid purpura abdominalis.
