不同冠状动脉CT影像特征的老年患者血清细胞因子水平差异

         

摘要

Objective To discuss the difference in serum cytokine levels in elderly patients with different features of coronary CT imaging. Methods The elderly people ( n = 224 ) with medical examination were selected and given coronary computed tomography angiography ( CTA ). According to plaque compositions and CT values, all patients were divided into calcified plaque group ( n = 31, CT value > 300 Hu ), non-calcified plaque group ( n = 53 , CT value < 150 Hu ), mixed plaque group ( n = 62, with lipid and fibrous plaques ) and control group ( n = 78 , without plaques after CTA ). The levels of interleukin-6 ( IL-6 ),IL-8, IL-10,GM-CSF, interferon-γ ( IFN-γ ), tumor necrosis factor-α ( TNF-α ) were detected by using flow fluorescence technique, and the difference in serum cytokine levels were compared in all groups. Results The differences in levels of IL-6 and IFN-γ had statistical significance among all groups ( P < 0.05 ),and those in levels of IL-8,GM-CSF,IL-10 and TNF-α had no statistical significance among all groups ( P > 0. 05 ). The levels of IL-6 and IFN-γ increased significantly in non-calcified plaque group and mixed plaque group compared with control group and calcified plaque group ( all P < 0.05 ). Conclusion There is a certain correlation between non-calcified plaque compositions and levels of serum IL-6 and IFN-γ. The stability of plaques can be determined through understanding the changes of cytokine compositions and coronary CTA findings, which is important to diagnosis, treatment and prognosis in clinic.%目的 探讨不同冠状动脉CT影像特征的老年患者血清细胞因子水平差异.方法 纳入我院老年体检人群224例,进行冠状动脉CTA检查,据斑块成分及CT值大小将受试者分为钙化斑块组(斑块CT值>300 Hu,n=31)、非钙化斑块组(斑块CT值<150 Hu,n=53例)、混合斑块组(含有脂质和纤维的斑块,n=62),对CTA结果核查确定无粥样斑块形成者作为对照组(n=78).流式荧光技术联检各组人群血清IL-6、IL-8、IL-10、GM-CSF、IFN-γ、TNF-α因子水平,并比较各组细胞因子水平的差异.结果 四组人群IL-6、IFN-γ水平各组间差异具有统计学意义(P<0.05);而IL-8、GM-CSF、IL-10、TNF-α水平,组间无统计学差异(P>0.05).非钙化斑块组与混合斑块组患者IL-6与IFN-γ水平较对照组及钙化斑块组显著升高(P均<0.05).结论 混合板块与非钙化板块的斑块成分与血清IL-6、IFN-γ等细胞因子存在一定关联,通过了解细胞因子成分变化辅助冠脉CTA检查结果,判断斑块的稳定性,对于指导临床诊断治疗和改善患者预后具有重要的意义.
