丹红注射液对慢性心力衰竭患者的影响

         

摘要

目的 探讨丹红注射液对慢性心力衰竭(CHF)患者的疗效和影响.方法 纳入武警后勤学院附属医院心内科于2014年4月~2015年12月治疗的CHF患者84例,男性47例,女性37例.按随机数字表法分为对照组(42例)和研究组(42例).对照组依据病情给予常规药物治疗,研究组在对照组治疗基础上予以丹红注射液.治疗前和治疗1周后测定内皮素-1(ET-1)、降钙素基因相关肽(CGRP)、一氧化氮(NO);肿瘤坏死因子-α(TNF-α)、白介素-6(IL-6)、白介素-1β(IL-1β)、核转录因子-κB(NF-κB);纤维蛋白原(FIB)、D-二聚体(D-D).心脏超声测定左室收缩末期内径(LVESD)、左室舒张末期内径(LVEDD)、左室内径(LVD)、左室射血分数(LVEF).评价临床疗效.结果 与治疗前比较,两组治疗后血浆ET-1含量降低,NO、CGRP含量升高,差异有统计学意义(P均<0.05).与对照组治疗后比较,研究组ET-1含量降低,NO、CGRP含量升高,差异有统计学意义(P均<0.05).两组治疗后较治疗前TNF-α、IL-6、IL-1β及NF-κB含量降低,差异有统计学意义(P均<0.05).与对照组治疗后比较,研究组TNF-α、IL-6、IL-1β及NF-κB含量降低,差异有统计学意义(P均<0.05).与治疗前比较,两组治疗后血浆FIB、D-D含量降低,研究组降低更明显,差异有统计学意义(P均<0.05).相对于治疗前,两组治疗后LVESD、LVEDD及LVD降低,LVEF升高,差异有统计学意义(P均<0.05).与对照组治疗后比较,研究组LVESD、LVEDD及LVD降低,LVEF升高,差异有统计学意义(P均<0.05).研究组临床疗效有效率为90.48%,高于对照组的71.43%,差异有统计学意义(P<0.05).结论 慢性心力衰竭患者在常规药物治疗基础上加用丹红注射液可改善血管内皮功能和纤溶系统,降低炎症反应,提高心功能.%Objective To discuss the curative effect and influence of Danhong Injection in patients with chronic heart failure (CHF).Methods CHF patients (n=84, male 47 and female 37) were chosen from the Department of Cardiovasology of Affiliated Hospital of Logistics University of Chinese People's Armed Police from Apr. 2014 to Dec. 2015, and then divided, according to random digital table, into control group and study group (eachn=42). The control group was treated with routing drugs according to patients' conditions, and study group, with Danhong Injection additionally. The indexes of endothelin-1 (ET-1), calcitonin gene-related peptide (CGRP), nitric oxide (NO), tumor necrosis factor-α (TNF-α), interleukin-6 (IL-6), interleukin-1β (IL-1β), nuclear factor kappa (NF-κB), fibrinogen (FIB) and D-dimer (D-D) were detected before and after treatment for 1 week. The indexes of left ventricular end-systolic diameter (LVESD), left ventricular end-diastolic inner diameter (LVEDD), left ventricular diameter (LVD) and left ventricular ejection fraction (LVEF) were detected. The curative effect was reviewed. Results The content of ET-1 decreased, and content of NO and CGRP increased in 2 groups after treatment (allP<0.05). Compared with control group, content of ET-1 decreased, and content of NO and CGRP increased in study group after treatment (allP<0.05). The content of TNF-α, IL-6, IL-1β and NF-κB decreased in 2 groups after treatment (allP<0.05). Compared with control group, content of TNF-α, IL-6, IL-1βand NF-κB decreased in study group after treatment (allP<0.05). The content of plasma FIB and D-D decreased in 2 groups, and decreased more significantly in study group after treatment (allP<0.05). The levels of LVESD, LVEDD and LVD decreased, and LVEF increased in 2 groups after treatment (allP<0.05). Compared with control group, the levels of LVESD, LVEDD and LVD decreased, and LVEF increased in study group after treatment (all P<0.05). The clinical effective rate was 90.48% in study group and 71.43% in control group (P<0.05).Conclusion In treatment of CHF, routine drugs combined with Danhong Injection can improve vascular endothelial function andfibrinolytic system, inhibit inflammatory reaction and promote heart function.
