乳腺癌中HSP90和Caspase-3的表达及其与临床病理、预后关系的研究

         

摘要

Objective To investigate the expression of HSP90 and Caspase-3 in breast cancer and the correlation with clinicopathologic characteristic and prognosis. Methods The expression of the HSP90 and Caspase-3 protein was detected with immunohistochemical Envision plus nonhiotin technique in 180 cases of breast cancer and 80 cases of pericarcinoma tissues. The correlation with clinicopathologic characteristic and prognosis were analyzed. Results The positive expression rates of HSP90 protein in breast cancer were significantly higher than that in the pericarcinoma tissues( 37. 2% vs. 7. 5% , P < 0.01 ),and the positive expression rates of Caspase-3 protein in breast cancer were significantly lower than that in pericarcinoma tissues( 23.9% vs 65.0% , P <0. 01 ). The expression of HSP90 were positively related to tumor size, tumor clinical stage, differentiation degree and lymph node metascasis( P <0. 01 ). The expression of the Caspase-3 were positively related to tumor clinical stage and lymph node metascasis( P <0. 01 ). The expression of the HSP90 was negatively correlated with the Caspase-3( r = -0.429, P <0. 01 ). Through Cox multivariate regression analysis, lymph node metascasis, tumor clinical stage, the expression of HSP90 and Caspase-3 were identified as independent factors associations with 5-year survival rate ( P <0.05 ). Conclusion The expression of the HSP90 was showed increase, while the expression of Caspase-3 was showed decrease in the breast cancer tissues. The expression of the HSP90 and Caspase-3 were of independent prognostic importance for prognosis in breast cancer patients. Furthermore, the HSP90 inhibited apoptosis control of breast cancer cells through inhibiting the expression of the Caspase-3.%目的 探讨乳腺癌中HSP90和Caspase-3的表达及其与临床病理、预后的关系.方法 采用免疫组织化学Envision 二步法检测HSP90和Caspase-3蛋白在180例乳腺癌组织和80癌旁组织中的表达,并分析其与乳腺癌临床病理因素及预后的关系.结果 HSP90在乳腺癌组织中表达阳性率明显高于癌旁组织(37.2% vs 7.5%,P<0.01),Caspase-3阳性表达率显著性低于癌旁组织(23.9% vs 65.0%,P<0.01).HSP90在乳腺癌组织中的表达与肿瘤大小、腋窝淋巴结转移、组织学分化和TNM分期密切相关(P<0.01),Caspase-3在乳腺癌组织中的表达与TNM分期和腋窝淋巴结转移密切相关(P<0.01).HSP90蛋白和Caspase-3蛋白在乳腺癌组织中的表达呈负相关(r=-0.429,P<0.01).Cox回归单因素分析显示腋窝淋巴结转移、组织学分级、肿瘤大小、TNM 分期、HSP90及Caspase-3阳性表达与乳腺癌患者5年生存率明显相关(P<0.05);Cox回归多因素分析表明腋窝淋巴结转移、TNM分期、HSP90及Caspase-3表达是乳腺癌患者5年生存率的独立预后因素(P<0.05).结论 乳腺癌组织中HSP90表达升高,而Caspase-3表达降低,两者表达均为乳腺癌预后的独立危险因素;HSP90可能通过某些途径抑制Caspase-3表达来抑制乳腺癌细胞的凋亡调控.
