环磷酰胺联合泼尼松治疗系统性红斑狼疮患者疗效及其对血清细胞因子的影响

摘要

Objective To observe the effect of prednisone combined with cyclophosphamide in patients with systemic lupus erythematosus interferon and interleukin.Methods From May 2012 to May 2016,in Sichuan Ganzi Tibetan Autonomous Prefecture People's Hospital,86 cases of inpatients in the treatment of systemic lupus erythematosus enrolled as the research object,they were randomly divided into control group (43 cases) and observation group (43 cases),the control group was given prednisone treatment,the observation group based on control group therapy,also used of cyclophosphamide,compared 2 groups of therapeutic effect of interferon and interleukin.Results The total effective rate of observation group was 93.02％,which was higher than 76.74％ in the control group(χ2=4.440,P=0.035);after treatment,the observation group of patients' IgA,IgG,IgM levels lower than the control group,C3,C4 levels higher than the control group,the differences were statistically significant (t=3.374,P=0.001;t=5.966,P=0.000;t=3.013,P=0.004);After treatment,IL-6,IL-10,IFN-α levels decreased,compared 2 groups of IL-6,IL-10 and IFN-α levels after treatment,the difference was statistically significant (t=6.779,P=0.000;t=4.734,P=0.000;t=7.990,P=0.000).Conclusion Cyclophosphamide combined with prednisone can improve the treatment effect greatly,and reduce the level of INF-α,IL-6,IL-10.%目的 观察环磷酰胺联合泼尼松治疗系统性红斑狼疮患者的疗效及其对血清细胞因子的影响.方法 选取2012年5月-2016年5月四川甘孜藏族自治州人民医院内三科住院治疗的系统性红斑狼疮患者86例作为研究对象,按照随机数字表法分为对照组和观察组各43例,对照组给予泼尼松治疗,观察组在此基础上加用环磷酰胺,比较2组治疗效果及对血清免疫球蛋白、补体C3、C4、IL-6、IL-10、INF-α水平的影响.结果 观察组总有效率为93.02％,显著高于对照组的76.74％(χ2=4.440,P=0.035);治疗后,2组患者IgA、IgG、IgM均降低,C3、C4均升高,且观察组患者的IgA、IgG、IgM水平低于对照组(t=3.374、5.966、3.013,P<0.05),C3、C4水平高于对照组(t=4.205、3.614,P<0.05);治疗后,2组患者IL-6、IL-10、INF-α水平均降低,且观察组IL-6、IL-10、INF-α水平较对照组低,差异具有统计学意义(t=6.779、4.734、7.990,P<0.05).结论 环磷酰胺与泼尼松联合应用可极大地提高系统性红斑狼疮的治疗效果,并降低IFN-α,IL-6、IL-10水平.
