血尿酸与代谢综合征关系的研究

         

摘要

目的 分析代谢综合征(MS)的临床及生化特征,并探讨血尿酸(SUA)与MS之间的关系.方法 2009年4~10月,在宁夏医科大学附属医院进行的宁夏职业人群MS流行病学基线调查人群中,采用病例对照的研究方法,将研究对象分为两组,MS病例组687例,对照组1357例.对两组SUA水平及异常率,以及MS相关指标水平进行比较,并分析SUA与相关指标水平的相关性,同时比较不同MS组分组SUA的水平,观察分析SUA与MS及各种组分之间的关系.结果 MS病例组SUA均值及高尿酸血症(HUA)患病率均明显高于对照组(t=10.817,x2=88.324,P<0.01);SUA水平与年龄、BMI、WC、SBP、DBP、TC、TG呈正相关(r分别为0.083,0.345,0.417,0.213,0.227,0.146,0.339,P<0.01),与HDL-C呈负相关(r=-0.249,P<0.01);不同MS组分间SUA水平差异有统计学意义(F=58.108,P<0.01).随着MS病例组分增多及SUA水平增高,性别、超重、肥胖、高血压、血脂紊乱为HUA的独立危险因素.以HUA为因变量,以代谢异常的个数为自变量进行logistic回归分析的结果显示,与对照组相比,代谢异常各亚组发生HUA风险逐步增加.结论 MS与SUA、向心性肥胖及脂代谢紊乱明显相关.因此,SUA水平升高是MS的一个重要危险因素,在MS的演变与进展中发挥着重要作用.%Objective To analyze the clinical and biochemical characteristics of metabolic syndrome (MS) and to explore the relationship between serum uric acid (SUA) and MS. Methods The data of the MS epidemiological investigation carried out in the Affiliated Hospital of Ningxia Medical University in April to September 2009 for Ningxia professional personnel were collected. By using the case-control study method, the subjects were divided into two groups, MS group (n=687) and control group (n=1357). The SUA levels,its abnormal rate and the MS related parameters between the 2 groups were compared. The correlation of SUA with MS related indices was analyzed. The correlation of SUA with the different numbers of MS components was observed. Results In the MS group, the mean value of SUA and the morbidity of hyperuricemia (HUA) were higher than those in the control group, (t= 10.817, X = 88. 324, P<0. 01). SUA levels were positively correlated with age, BMI, WC, SBP, DBP, TC, and TG (r =0. 083, 0. 345, 0. 417, 0. 213, 0. 227, 0. 146, 0. 339, P<0. 01) , and were negatively correlated with HDL-C (r= —0. 249, P<0. 01). There was a significant difference in the SUA levels among groups with different numbers of MS components (F=58. 108, χ2 =116. 131, P<0. 01). With the increase of MS components and SUA levels, the gender, overweight, obesity, hypertension, and patholipidemia became the independent risk.factors of HUA. In the logistic regression analysis, HUA was taken as the dependent variable and the number of abnormal metabolism as the independent variable, and the result was that as compared with the control group, the prevalence of HUA in the abnormal metabolism subgroups was increased gradually. Conclusions MS is significantly associated with SUA, central obesity, and dyslipidemia. Therefore, the increase of SUA is an important risk factor for MS, and it plays an important role in the occurrence and progress of MS.
