目的 探讨地特胰岛素(Det)在T2DM患者围手术期的疗效.方法 60例围手术期T2DM患者随机分为Det加门冬胰岛素组30例,精蛋白人胰岛素(NPH)加人胰岛素组30例.观察两组在治疗后血糖、血糖调整时间、平均住院时间的差异.结果 三餐前血糖、术后第1天血糖、血糖调整时间和平均住院时间Det组与NPH组差异有统计学意义;而两组间低血糖发生、胰岛素用量差异无统计学意义.结论 Det能迅速、有效、乎稳控制血糖,对T2DM围手术期患者BG达标明显优于NPH皮下注射,是目前围手术期控制血糖的良好方法.%Objective To discuss the therapeutic effect of insulin detemir in perioperative patients with type 2 diabetes. Methods Sixty peri-operative patients with type 2 diabetes were treated with insulin detemir plus insulin aspart injection (D group, n=30) and neutral protamine hagedorn insulin plus biosynthetic human insulin injection (NPH group, n=30). Their blood glucose, blood glucose adjustment time, length of hospitalization, and incidence of hypoglycemia were observed. Results The levels of fasting blood glucose, blood glucose adjustment time, fasting blood glucose of postoperation, and the average length of hospitalization were significantly lower in the D group than in the NPH group (P< 0. 05). The incidence of hypoglycemia and the dose of insulin of the two groups were not statistically different. Conclusion Insulin detemir is superior to NPH in controlling blood glucose of perioperative patients with type 2 diabetes steadily and swiftly. It is an effective way for perioperative patients in controlling blood glucose at present.
