药物超敏反应综合征致暴发性1型糖尿病1例报道及文献复习

摘要

Objective To investigate the clinical characteristics and mechanisms of fulminant type 1 diabetes mellitus (FT1DM) caused by drug-induced hypersensitivity syndrome (DIHS) and to provide references for clinical practice.Methods One case of FT1DM caused by DIHS in our hospital was reported.Relevant literatures were retrieved in PubMed from 1950 to 2016.Results According to the literatures and the case report in our hospital,a total of 22 FT1DM patients caused by DIHS were collected and summarized.All patients had medication history with positive Human herpesvirus 6(HHV-6) in 14 cases.Carbamazepine,mexilletine and allopunnol were the most common allergic drugs.Glucocorticoids treatment for FT1DM was effective.Conclusion DIHS can induce FT1DM.Blood glucose should be monitored when DIHS occurs.%目的 探讨药物超敏反应综合征(DIHS)引起暴发性T1DM(FT1DM)的临床特点、原因及致敏药物,为临床诊治提供参考.方法 报道我院1例DIHS致使FT1DM病例,并通过检索1950~2016年发表在PubMed的相关文献进行分析.结果 综合文献报道以及我院DIHS致FT1DM 22例,14例患者的人疱疹病毒6型(HHV-6)阳性,多见于卡马西平、美西律、别嘌醇药物激发引起,治疗上可使用糖皮质激素.结论 DIHS可导致FT1DM的发生,出现DIHS时,注意监测血糖.
