257例肛门及肛管尖锐湿疣组织中HPV感染的基因分析

摘要

Purpose To study the genotypes of human papillomavirus ( HPV ) and its clinical characteristics in condyloma acuminate ( CA ) of anus and anal canal in 28 years. Methods 23 types of HPV genotype were detected in 257 patients with CA of anus and a-nal canal by polymerase chain reaction ( PCR ) and gene-chips technique, and the related clinical pathology data of patients were analyzed. Results There were 183 positive cases in 257 patients with CA of anus and anal canal and the ratio of total HPV infection was 71. 21% ( 183/257 ). The ratio of single types was 54. 09% ( 139/257 ) and multiple types was 17. 12% ( 44/257 ). The predominant type of single infection is HPV6 and the ratio is 25. 68% ( 66/257 ), followed by HPV 11 ( 24. 90% , 64/257 ). Otherwise, the predominant type of multiple infection is HPV6 + 11, the ratio was 52. 27% ( 23/44 ), and HPV6 + 11 + 16 was 6. 82% ( 3/44 ). Conclusions It is HPV 6, 11, 6 + 11, 6 + 11 +16 that are the main genotypes in the pathogenesis of CA with anus and anal canal. Gene-chips technique is a high sensitive and specific method in the detection and typing of high or low HPV.%目的 探讨肛门及肛管尖锐湿疣病变中人乳头瘤病毒(human papillomavirus,HPV)感染的基因型别及临床意义.方法 从257例肛门及肛管尖锐湿疣(condyloma acuminata,CA)石蜡组织标本中提取23种HPV DNA,采用基因扩增结合基因芯片技术对其组织的23种HPV基因型别进行检测,并对其患者进行临床病理资料分析.结果 257例肛门及肛管CA病变组织中检出HPV阳性者183例,HPV感染率为71.21%(183/257).其中一重HPV感染139例,阳性检出率为54.09%(139/257),多重HPV感染44例,阳性检出率为17.12%(44/257).一重HPV感染中HPV6型66例,阳性检出率为25.68%(66/257),HPV6型是最主要的感染型别;其次是HPV11型,共有64例,其阳性检出率为24.90%(64/257).多重HPV感染中,HPV6+11型23例,占多重感染的52.27%(23/44),是多重感染的主要型别;其次是HPV6+11+16型3例,占多重感染的6.82%(3/44).结论 HPV6型、11型、6+11型、6+11+16型感染是肛门及肛管CA的主要致病类型.基因扩增结合基因芯片技术是一种比较适合临床进行高、低危HPV分型检测的诊断方法,其敏感性高、特异性好.
