目的 对不同付费方式下住院费用进行分析,指出费用差异原因,提出合理控制住院费用的思路.方法 以老年病人肺部感染为例,对7类病人住院费用进行分析比较.结果 7类病例住院天数、总费用、床日费均见显著性差异.住院费用与住院天数、药品比例、付费方式、出院方式4个因素有显著相关性.结论 推进构建城乡一体化基本医疗保障制度,从制度上规范医疗行为:同时医疗机构应加强自律,在用药以及诊治的各个环节控制医疗费用.%Objective To analyze the hospitalization costs using different payment methods, point out the reasons for cost differences and put forward the idea of the reasonable control of the hospitalization costs. Method Taking pulmonary infection for example, the costs of seven classes of patients were analyzed and compared. Result There had significant differences among these seven classes of patients in the number of days of hospitalization, total cost, daily cost per bed. Hospital costs had significant correlation with length of hospital stay, pharmacy ratio, payment methods, and discharged ways. Conclusion Advancing to build a basic medical insurance system for urban and rural integration can regulate medical practices from control system aspect. Meanwhile, medical institutions should strengthen self — discipline to control medical expenses at all aspects of pharmacy, diagnosis and treatment processes.
