心理干预对肺癌患者焦虑抑郁症状影响的Meta分析

摘要

Objective To investigate the effect of psychological intervention on anxiety and depression symptoms of patients with lung cancer using meta - analysis. Methods Wan Fang, CNKI, VIP and CBM databases were searched for clinical controlled trials on effect of psychological intervention on anxiety and depression symptoms of patients with lung cancer. Data were analyzed with RevMan 5. 1. 6 and Stata 12. 0 softwares. Results were described as standardized mean difference or odds ratio with 95% confidence intervals. Results A total of 8 clinical controlled trials involving 608 lung cancer patients ( 349 in intervention group and 259 in control group ) were included in this systematic review. Meta - analysis showed that there was no significant difference between the two groups in pre - intervention self - rating depression scale scores and self - rating anxiety scale scores. However, post - intervention self-rating depression scale scores and self - rating anxiety scale scores of the intervention group were significantly lower than that of the control group ( P≤0. 05 ). Conclusion The present data suggest that psychological intervention may considerably improve anxiety and depression symptoms and quality of life of patients with lung cancer. Further well - designed randomized controlled trials are needed to confirm current conclusions.%目的 采用Meta分析的方法探讨心理干预对肺癌患者焦虑抑郁症状的影响.方法 计算机检索万方、维普、CNKI及中国生物医学等中文数据库,全面收集有关心理干预对肺癌患者焦虑抑郁症状影响的临床对照试验,汇总数据采用RevMan 5.1.6和Stata 12.0进行分析,两组之间差异采用标准均数差(SMD)或OR及其95%CI描述.结果 根据纳入和排除标准最终纳入8个临床对照试验,包括608例肺癌患者,其中观察组349例,对照组259例.Meta分析结果表明:治疗前,心理干预组与对照组患者抑郁自评量表(SDS)评分[ SMD=0.61,95%CI(-0.63,1.86),P=0.33]和焦虑自评量表(SAS)评分[SMD=-0.01,95%CI(-1.08,1.05),P=0.98]差异均无统计学意义(P＞0.05);治疗后,心理干预组患者SDS评分[SMD=-1.03,95%CI(-1.64,-0.43),P=0.0007]和SAS评分[ SMD=-1.11,95%CI(-2.24,0.02),P=0.05]低于对照组患者,两组之间差异有统计学意义(P≤0.05).结论 现有证据表明,心理干预可有效地改善肺癌患者的焦虑抑郁症状,改善患者生活质量.由于纳入研究质量及数量有限,本研究结论尚需大样本高质量临床研究进行证实.
