自制新型皮肤保温装置在冷冻消融术中应用的有效性及安全性研究

摘要

Objective:To probe into the effectiveness and safety of a self made new type of skin thermal insula-tion device for patients undergoing freeze ablation.Methods:A total of 80 cases of tumor patients with cryoabla-tion therapy were randomly divided into control group and observation group,40 cases in each.The patients in control group used the traditional warm saline gloves;the patients in observation group adopted new skin insula-tion device.The patients were observed and compared on the intraoperative temperature,stress indicators and postoperative general situation,incidence of surgical complications and surgical satisfaction between both groups.Results:Before and after the start of surgery,30min and 10min after the start,the body temperature was not significantly different between both groups;after the end,the body temperature in observation group was significantly higher than that in control group,and the difference was statistically significant(P<0.05);At the end of the operation,the levels of blood glucose,insulin and cortisol in observation group were higher than those before anesthesia,and they in control group was higher than those in observation group(P<0.05).The nursing time and hospitalization time in observation group were shorter than that in control group(P<0.05).The inci-dence of palpitation,edema,blood pressure and heart rate was lower in observation group than that in control group(P<0.05).The satisfaction rate of patients in observation group was significantly higher than that in con-trol group(P<0.05).Conclusion:The application of self made new type of skin thermal insulation device in cryoablation could significantly reduce the incidence of complications,reduce the stress response,and had the value of clinical application.%[目的]探讨自制新型皮肤保温装置在冷冻消融术中应用的有效性和安全性。[方法]将80例接受冷冻消融治疗的肿瘤病人随机分为对照组和观察组各40例，对照组采用传统温盐水手套，观察组采用新型皮肤保温装置，观察比较两组病人术中体温、应激指标、术后一般情况、手术并发症发生率及手术满意度的差异。[结果]两组病人冷冻消融术开始前、开始后10 min和开始后30 min的体温无明显差别(P>0.05)，观察组病人手术结束后的体温明显高于对照组(P<0.05)；手术结束时观察组病人血糖、胰岛素、皮质醇水平均较麻醉前增高，且对照组高于观察组(P<0.05)；观察组病人的护理时数、住院时间较对照组短(P<0.05)；观察组病人皮肤苍白、水肿、血压降低和心率细速发生率低于对照组(P<0.05)；观察组病人手术满意率明显高于对照组(P<0.05)。[结论]自制新型皮肤保温装置在冷冻消融术中的应用可明显降低病人并发症发生率，减轻病人应激反应。
