高危型HPV检测联合细胞学检查在宫颈病变筛查中的应用

摘要

目的：探讨高危型HPV检测联合细胞学检查在宫颈病变筛查中的的意义。方法：选取在笔者所在医院进行宫颈癌筛查检测的女性患者1875例，对所有患者分别进行高危型HPV基因检测(HR-HPV-DNA)、宫颈液基薄层细胞检测(TCT)、HR-HPV-DNA+TCT联合检查，以病理学检验为确诊依据。结果：在不同等级的宫颈病变中，HR-HPV+TCT检查的阳性率不同，宫颈癌前病变(CIN)Ⅰ级患者中为62.12%、CINⅡ患者中77.63%、CINⅢ患者中83.33%、宫颈浸润癌患者中92.31%，总敏感度为97.33%，特异度为71.20%。结论：高危型HPV检测联合细胞学检查应用于宫颈病变筛查中，可以增加筛查的有效率，适用于宫颈癌的早期诊断。%Objective:To investigate the significance of the detection of high-risk HPV combined with cytological examination in cervical lesions screening. Method:1875 cases of female patients,screening for cervical cancer.All of the patients,respectively for HPV type gene detection(HR HPV-DNA),cervical liquid thin layer cell test(TCT),HR-HPV DNA+TCT joint inspection,pathological diagnosis standard for inspection.Result:In different levels of cervical lesions, HR-check out the positive rate of HPV+TCT was different.CIN Ⅰ patients was 62.12%,CIN Ⅱ patients was 77.63%,CIN Ⅲpatients was 83.33%,92.31%of patients with cervical invasive cancer.The overall sensitivity was 97.33%.Specific degree was 71.20%.Conclusion:High-risk type HPV detection combined cytological examination,used in screening of cervical lesions,can increase the screening efficiency,is suitable for early diagnosis of cervical cancer.
