2型糖尿病患者中幽门螺杆菌感染与胰岛素抵抗的相关性分析

摘要

目的：探讨2型糖尿病患者中幽门螺杆菌(Hp)感染与胰岛素抵抗的关系。方法：选择2型糖尿病患者73例，根据有无Hp感染分为两组，A组(2型糖尿病合并Hp感染组)48例与B组(2型糖尿病无Hp感染组)25例。分别测定血浆超敏C反应蛋白、空腹血糖、空腹胰岛素、肝肾功能、血脂、糖化血红蛋白、血浆胃促生长素、YY肽(PYY)等指标，计算稳态模型胰岛素抵抗指数(HOMA-IR)，比较其水平的差异，并进行相关性分析。结果：两组患者的临床资料及血清hs-CRP等的检测结果可见，A组BMI、腰臀比、空腹血糖、甘油三酯、hs-CRP、HOMA-IR及血浆胃促生长素均增高，YY肽下降，与B组比较差异有统计学意义(P<0.05)。结论：Hp感染可能在2型糖尿病胰岛素抵抗的发生中发挥了重要的作用。%Objective:To investigate the correlation of Helicobacter pylori in patients with type 2 diabetes infection and insulin resistance.Method:73 cases of type 2 diabetic patients according to whether Hp infection were divided into two groups,group A (48 cases) and gruop B (25 cases).The plasma high-sensitivity C-reactive protein,fasting glucose,fasting insulin,liver and kidney function,blood lipids,HbA1c,plasma ghrelin,YY peptide and other indicators were measured,and homeostasis model assessment of insulin resistance index(HOMA-IR).Comparing differences in their levels and correlation analysis. Result:The level of BMI,WHR,FBG,2 h PG,FINS,TG,HOMA-IR,hs-CRP and plasma ghrelin of group A were higher than group B,YY peptide decreased,the differences were statistically significant(P<0.05).Conclusion:Hp infection may play an important role in the occurrence of type 2 diabetes insulin resistance.
