黄芪桂枝五物汤化裁内服、熏洗结合功能训练对手外伤术后患者上肢功能及徒手肌力的影响

摘要

目的：观察黄芪桂枝五物汤化裁内服、熏洗结合功能训练治疗手外伤术后患者的效果，重点研究其对患者上肢功能及徒手肌力的影响。方法：收治手外伤术后患者104例，随机平分为两组。对照组给予常规西医抗菌、清创、抗感染等治疗，并指导患者进行常规手部功能康复训练；观察组则在康复功能训练的基础上给予由黄芪桂枝五物汤化裁的中药汤剂内服、熏洗治之，两组均以1个月为1个疗程。用上肢功能指数量表(UEFI)对治疗前后上肢功能的变化进行评估，并评价患者治疗前后徒手肌力的变化情况，观察机体血液流变学的改善情况。结果：患者上肢功能经治疗后有所提高，UEFI评价与治疗前相比升高明显，观察组患者升高幅度明显大于对照组(P＜0.05)；患者经治疗后徒手肌力改善明显，观察组改善较对照组更突出(P＜0.05)；两组患者血液流变学有不同程度的改善，观察组的相关指标改善更突出(P＜0.05)。结论：黄芪桂枝五物汤化裁内服、熏洗结合功能训练利于提高手外伤术后患者的上肢功能，改善徒手肌力，是中西医结合治疗手外伤术后患者的突出成果。%Objective:To observe the effects of huangqi guizhi wuwu tang taken orally and the fumigating and washing combined with functional training on the patients with hand injury after operation,the key research is the effects on the patients’ upper limb function and manual muscle strength.Methods:104 patients after the operationg of hand injury were hospitalized,and they were divided into two equal groups randomly.The patients in the control group were given the treatment with conventional Western medicine antibacteria,debridement,anti-infection and so on,meanwhile they were taught to take the conventional rehabilitation training of hand function,and those in the observation group were given the treatment with the herbal decoction of huangqi guizhi wuwu tang and the fumigating and washing based on the treatment with rehabilitation function training.The course of treatment of the two groups were both one month.The circumstance of upper limb function changes between before and after treatment were estimated by upper libmb function exponent index(UEFI),also the circumstance of manual muscle strength changes between before and after treatment were estimated,the improved conditions of organism blood rheology were observed.Results:the patients'upper limb funtion was improved after treatment,and the UEFI evaluation was improved remarkablely compared with the index before treatment.The ascending range of patients in observation group was much greater than those in control group(P<0.05).The manual muscle strength of patients was improved obviously after treatment,and the improvement of patients in observation group was more outstandingly(P<0.05).The blood rheology of the two groups was ameliorated in varying degress,and the relevant indices of the patients in observation group were more clearly(P<0.05).Conclusion:It is beneficial to iprove the patients' upper limb function after the operation of hand injury and promote manual muscle strength by the huangqi guizhi wuwu tang oral administration and the fumigating and washing combined with functional training.It's a outstangding achievement of diagnosing and treating patients after the operation of hand injury in combined treatment of traditional Chinese and Western medicine.
