我院住院药房用药错误分析及防范措施

摘要

Objective To reduce medication errors and improve the quality of drug dispensing, so as to ensure the medication safety of patients in inpatient pharmacy. Methods Totally 239 cases of medication errors in inpatient pharmacy in our hospital from 2013 to 2015 reported to Monitoring Network of Medication Safety in China Core Group of International Network for the Rational Use of Drugs (INRUD) were classified and analyzed. Results The common medication errors were drug dosage(23. 85％ ), frequency of drug adminis-tration(20. 08％ ) and varieties of drugs(20. 08％ ), among which the main mistakes were caused by primary pharmacists(83 cases) and primary doctors(47 cases). Conclusion The standardized management of drug in the hospital pharmacy and the establishment of a double check system can reduce the occurrence of medication errors.%目的 减少住院药房用药错误,提高药品调配质量,保障患者用药安全.方法 对医院2013年至2015年住院药房上报《INRUD中国中心组临床安全用药监测网》的239例用药错误进行统计和分析.结果 常见的用药错误类型为药品用量(23.85％)、给药频次(20.08％)、药品品种(20.08％),其中主要引起错误的人员类型为初级药师(83例)和初级医师(47例).结论 加强住院药房药品的标准化管理,制订双人核对制度等措施,可减少用药错误的发生.
