一次性包皮环切缝合器与传统包皮环切术的临床疗效比较研究

摘要

Objective To compare the clinical efficacy of stapler circumcision and conventional circumcision in patient with redundant prepuce or phimosis.Methods Totally 210 patients with redundant prepuce or phimosis who underwent stapler circumcision (suture group,122 cases) and conventional circumcision (conventional operation group,88 cases) from March 2013 to March 2015 were enrolled and followed up for 1-2 months.The operation duration,intraoperative blood loss,intraoperative and postoperative 24 h pain visual analogue scale (VAS) score,healing time,incidence of complications and satisfaction rate of appearance.Results The operation duration,intraoperative blood loss,intraoperative and postoperative 24 h VAS scores,healing time,incidence of complications were significantly lower,the satisfaction rate of appearance was significantly higher in stapler group than those in conventional surgery group [(7.1 ± 1.5) min vs (23.6 ± 5.2) min,(2.3 ± 1.2) ml vs (15.7±2.6) ml,(0.8±0.8) scores vs (2.4±1.2) scores,(1.8 ±1.0) scores vs (4.9±1.4) scores,(13 ±4) dvs(16±3) d,4.1％ (5/122) vs 18.2％ (16/88),96.7％ (118/122) vs86.4％ (76/88)](P ＜ 0.05).Conclusion Stapler circumcision has shorter operation duration,less blood loss,less pain,lower incidence of complications and higher satisfaction rate of appearance compared with conventional circumcision.%目的 比较一次性包皮环切缝合器与传统环切术治疗成人包皮过长及包茎的临床疗效.方法 对2013年3月至2015年3月于辽宁省大连市中心医院行包皮手术的210例成年患者进行分析,其中122例采用一次性包皮环切缝合器(缝合器组),88例采用传统包皮环切术(传统手术组),所有患者术后随访1～2个月,比较2组患者的手术时间、术中出血量、术中及术后24h视觉模拟评分法(VAS)评分、伤口愈合时间、术后并发症发生率及外观满意度.结果 缝合器组手术时间、术中出血量、术中及术后24 h VAS评分、伤口愈合时间、并发症发生率均低于传统手术组,外观满意度高于传统手术组,差异均有统计学意义[(7.1±1.5)min比(23.6±5.2) min、(2.3±1.2)ml比(15.7±2.6)ml、(0.8±0.8)分比(2.4±1.2)分、(1.8±1.0)分比(4.9±1.4)分、(13±4)d比(16±3)d、4.1％ (5/122)比18.2％ (16/88)、96.7％(118/122)比86.4％(76/88),P＜0.05].结论 一次性包皮环切缝合器较传统环切术手术时间短、术中出血少、痛苦小、并发症少、外观满意度高.
