目的:探索安全、有效、易行的胃管固定方法,减少胃管脱出和吻合口瘘并发症的发生.方法:将我科2009年1月～2011 年1 月202 例食管癌患者随机分为试验组与对照组,分别采用改良胃管固定法和传统胶布固定方法.结果:试验组胃管脱出发生率(5.94%)低于对照组(17.82%)(P<0.01),胃管重置试验组(2.97%)低于对照组(14.85%)(P<0.001),试验组皮肤及黏膜损伤发生率(4.95%)低于对照组(14.85%)(P<0.05)和吻合口瘘发生率(1.98%)低于对照组(8.91%)(P<0.05).两组胃管固定术后舒适度评分,术后1、4 和7 d 试验组均优于对照组(P<0.01).结论:改良胃管固定方法较传统胶布固定牢靠、舒适,降低了胃管脱出率和吻合口瘘等并发症的发生.%Objective: To explore the safe, effective and easy method of gastric tube fixed in order to reduce the incidence of gastric tube prolapse and anastomotic fistula. Methods: 202 cases of patients with esophageal cancer in our department from January 2009 to January 2011 were randomly divided into experiment group and control group, which respectively were given the improved fixation of the esophageal gastric tube and traditional method with tape. Results: The gastric tube extrusion rate of the experiment group (5.94%) was lower than that of the control group (17.82%) (P<0.01), there was a significant difference on the tube replacement (2.97% vs 14.85%) (P<0.001), skin and mucosal injury rate (4.95% vs 14.85%) (P<0.05) and anastomotic leakage (1.98% vs 8.91%) (P<0.05) in the two groups, two gastric tube fixation comfort score after 1 day, 4 days and 7 days, the experiment group were better than those of the control group (P<0.01). Conclusion: The improved fixation of gastric tube compared with the traditional method is fixed and comfortable, reduces the rate of complications such as gastric tube extrusion rate and anastomotic fistula.
