Objective: To investigate the effects of Hemabate on postpartum hemorrhage and lactation after cesarean section.Methods: 60 cases of pregnant women with uterine inertia after cesarean section were randomly divided into control group and observation group, each group had 30 cases.Patients in control group were treated with intravenous infusion of Oxytocin 20 U, Oxytocin 20 U im and Misoprostol 600 μg anal plug, while patients in observation group were treated with intravenous infusion of Oxytocin 20 U and Hemabate 250μg im.The amount of bleeding during operation and postoperative 24 h, preoperative and 24 h postpartum prolactin (PRL) concentration , the time after initiating lactation and postpartum 24 h, 48 h lactation score were observed.Results: The amount of bleeding during operation and postoperative 24 h, the time after initiating lactation in observation group were (221.34±56.49) ml, (342.51 ±67.85) ml, (21.36±2.36) h, significantly less than those in control group which were (249.87±65.47) ml, (387.54±76.37) ml, (26.54±2.48) h (P＜0.05); postpartum 24 h PRL concentration in observation group [(261.34 ± 88.74) μg/L]was significantly higher than that in control group [(248.67±87.59) μg/L, P＜0.05]post-24 h, 48 h lactation scores in observation group were 1.58±0.82, 1.97±0.94, significantly higher than those (1.24±0.76, 1.56±0.91) in control group (P＜0.05).Conclusion: Hemabate has better therapeutic effect on postpartum hemorrhage caused by uterine inertia, and improves postoperative lactation.%目的:探讨欣母沛防治剖宫产术后出血的疗效和对泌乳功能的影响.方法:将60例有宫缩乏力高危剖宫产孕妇随机分为对照组和观察组,每组30例.对照组胎儿取出后缩宫素20 U静滴+缩宫素20 U子宫肌内注射+米索前列醇600 μg肛塞,观察组胎儿取出后缩宫素20 U静滴+欣母沛250 μg子宫肌内注射.观察两组术中、产后24 h出血量,记录术前、产后24 h催乳素(PRL)浓度以及术后泌乳始动时间,产后24 h、48 h泌乳量评分.结果:观察组术中、产后24 h出血量、术后泌乳始动时间分别是(221.34±56.49)ml、(342.51±67.85)ml、(21.36±2.36)h,明显少于对照组的(249.87±65.47)ml、(387.54±7637)ml、(26.54±248)h,P<0.05;观察组产后24 h PRL浓度为(261.34±88.74)μg/L,明显高于对照组的(248.67±87.59)μg/L,P<0.05;观察组产后24 h、48 h泌乳量评分分别是1.58±0.82、1.97±0.94,显著高于对照组的1.24±0.76、1.56±0.91,P<0.05.结论:欣母沛显著减少剖宫产后出血,改善术后泌乳功能.
