目的 探讨脑卒中患者医院社康中心防治一体化护理管理模式的可行性,为脑卒中患者的护理提供科学的管理模式.方法 选择2008年3月～2011年2月在我院接受治疗的67例脑卒中患者,随机将其分为对照组和实验组.对照组33例给予住院常规护理管理,实验组34例给予防治一体化护理管理,比较两组患者在出院前对脑卒中相关疾病知识的了解程度、护理工作的满意度、疾病的控制情况.结果 实验组行一体化护理管理模式后,患者对疾病相关知识的掌握率为76.5%,理解率为14.7%,满意度为82.4%,基本满意度为17.6%;对照组分别为12.1%、39.4%、39.4%和48.5%,两组患者上述指标比较差异均有统计学意义(P ＜ 0.05或P ＜ 0.01).结论 脑卒中患者医院社康中心防治一体化护理管理模式是可行的,有助于患者控制病情,提高生活质量.%Objective To study the feasibility of integrated care management model of hospital community health center patients with stroke prevention and provide the scientific management model for stroke patients care. Methods 67 patients with cerebral apoplexy were chosen from March 2008 to February 2011 in our hospital were randomly divided into the control group (33 cases) and the experiment group (34 cases), the patients in control group were given conventional care management, and the experiment group were given the integration nursing management, the degree of knowledge to stroke, satisfaction of nursing, the control of the disease before discharge in two groups were compared. Results The rate of related knowledge of disease, understand rate, satisfaction rate and basic satisfaction rate in the experiment group after integrated care management model was 76.5%, 14.7%, 82.5%, and 12.1%, 39.4%, 39.4%, 48.5% in control group, respectively. The difference were statistically significant in two groups (P < 0.05 or P < 0.01). Conclusion It is feasible for stroke patients to obtain the hospital community health center prevention integrated nursing management mode, and help patients to control the disease, improve their quality of life.
