Objective To discuss monitoring service mode of general medical team to 2 type glycuresis in community.Methods 150 cases of patients with 2 type diabetes of the community from March 2011 to September 2012 were chosen as observation group.Patients and family were monitored by the general medical team, and had provided comprehensive intervened by diversified services.At the same time, another 150 patients were selected as control group, which had intervened by the general publicity, and monitor project were free detected.Two groups of weight, triglyc-eride, fasting blood glucose, and glycosylated hemoglobin, creatinine of before and after the intervention, and recognition rate of diabetes, therapy rate and control rate were compared.Results After intervened, the weight, triglyceride, fasting blood glucose, glycosylated hemoglobin, serum creatinine of patients were obvious dropt in observation group.Compared with before interventio, there were obvious difference (all P < 0.05).Compared with the control group of after intervention, there were obvious difference (all P < 0.05).Recognition rate were 98.6%, therapy rate were 97.3% and control rate were 90.0% in observation group.Compared with the control group, there were obvious differences (all P < 0.01).Conclusion The monitoring service mode of general medical team to 2 type glycuresis in community, can effectively controll the status of diabetic.It can provide operational experience for the community who has carried out diabetes standardized management.%目的 探讨全科医护团队对社区2型糖尿病监控服务模式.方法 选取2011年3月～2012年9月广东省深圳市民治社区150例2型糖尿病患者为观察组,以全科医护团队为患者及家庭进行监控,提供多元化的服务等综合干预,同时另选取150例患者为对照组,按一般的宣教,监控项目自由检测.比较两组患者干预前后体重、三酰甘油、空腹血糖、糖化血红蛋白、肌酐变化情况,和患者对糖尿病的认知率,治疗率及控制率.结果 干预后,观察组患者体重、三酰甘油、空腹血糖、糖化血红蛋白、肌酐都明显下降,与干预前比较,差异均有统计学意义(均P＜ 0.05),与干预后对照组比较,差异均有统计学意义(均P＜0.05).观察组患者认知率为98.6％,治疗率为97.3％,控制率为90.0％,与对照组比较,差异均有高度统计学意义(均P＜0.01).结论 全科医护团队对社区2型糖尿病监控服务模式,能有效地控制糖尿病患者病情,为社区开展糖尿病规范化管理提供可操作性的经验.
