螺旋断层放射治疗系统根治性放射治疗29例寡转移非小细胞肺癌的临床效果研究

         

摘要

Objective:To research the curative effect and adverse reaction of using helical tomotherapy (TOMO) on the radical radiotherapy for primary lesion and all of metastases of patients with non-small cell lung cancer of oligometastasis.Methods: 29 patients with NSCLC of oligometastasis (the number of metastases≤5) in initial treatment were enrolled in this retrospective analysis. The primary lesion and all of metastases were taken radical radiotherapy by using TOMO system, and the average and median of biological effective dose (BED10) 95Gy and 94Gy, respectively, and the number of neutral position chemotherapy period was 4.Results: In 30 months of neutral position follow-up visit, the total effective rate of lesion treatment in short-term curative effect was 80%, and the survival rates after 1, 2 and 3 years, respectively, were 52%,17% and 7%. The median survival time was 30 months, the progression-free survivals were 52%, 17% and 7% after 1, 2 and 3 years, and the median progression-free time was 12 months. The adverse reactions of most patients were 1-2th degree, and only a few patients were 3th degree. And the 4th degree of adverse reaction or the death related with treatment hadn't were found.Conclusions: Based on systematic therapy for non-small cell lung cancer of oligometastasis, the radical radiotherapy of primary lesion and all metastases can obviously improve patients' overall survival and progression-free survival, at the same time, its adverse reaction is durable during treatment.%目的:研究应用螺旋断层放射治疗系统(TOMO)根治性放射治疗寡转移非小细胞肺癌(NSCLC)患者原发灶及所有转移灶的疗效和不良反应.方法:回顾性分析29例初治转移灶≤5个的寡转移NSCLC患者,应用螺旋断层调强放射治疗系统对原发灶及所有转移灶行根治性放疗,平均和中位生物有效剂量(BED10)分别为95 Gy和94 Gy,中位化疗周期数4个.结果:中位随访时间30个月,近期疗效中病灶治疗总有效率为80%,1年、2年和3年的生存率分别为93%、69%和34%;中位生存时间为30个月,1年、2年和3年的无进展生存率分别为52%、17%和7%,中位无进展时间为12个月.多数患者仅出现1~2级不良反应,仅少数患者出现3级不良反应,未出现4级不良反应及治疗相关性死亡.结论:螺旋断层TOMO对寡转移NSCLC患者原发灶及所有转移灶均行根治性放疗,可以显著改善患者的总生存和无进展生存情况,治疗中的不良反应可耐受.
