27例严重急性呼吸综合征患者康复期运动心肺功能的动态观察

         

摘要

[Objective] To investigate the integral kinesthetic response of lung, heart, blood vessels, and muscles in SARS patients during recovery. [Methods] To observe cardiopulmonary exercise testing and the resting pulmonary function of 27 patients with critical severe acute respiratory syndrome in the 1st, 2nd, 3rd month after discharge. [Results] In the first test, the abnormality rates of cardiopulmonary exercise testing including VO2max%, VO2max/kg, VO2max, METs, BR, HRR were higher. Except that of VO2max%, the abnormality rates of other descriptors were higher than those of resting testing. With the recovery process, the abnormal rates of all the descriptors descended, and the disappearance of clinical symptoms was coincided with the recovery of cardiopulmonary exercise testing. [ Conclusions] The patients of SARS have the dysfunction of cardiopulmonary exercise testing, which may be related with the injury of cardiopulmonary system from SARS-CoV and a few exercise. But the cardiopulmonary function can recover gradually.%目的系统评估SARS患者康复期肺、心血管和肌肉等的综合运动反应能力.方法于患者出院后1、3、6个月时,分别对27例SARS-IgG阳性的SARS患者的运动心肺功能、静息肺功能和胸部CT进行动态观察.结果在第一次检查时,除VO2max%外,SARS患者运动心肺功能的VO2max%、VO2max/kg、VO2max、METs、BR、HRR及VEmax等各项指标的异常率均高于静息肺功能和CT检出的异常率.在第二、三次检查时,各项指标的异常率均明显下降,而且SARS患者的临床症状消失与运动心肺功能恢复相符合.结论SARS患者存在运动心肺功能障碍,可能与其心肺、肌肉等的损伤、发病后活动减少有关.SARS患者运动心肺功能障碍可以逐渐恢复.
