消瘾方加减联合依巴斯汀治疗慢性荨麻疹疗效评价及对IgE、IL－4和IFN－γ水平的影响

摘要

目的：：评价消瘾方加减联合依巴斯汀治疗慢性荨麻疹的临床疗效及对患者IgE、IL－4和IFN－γ水平的影响。方法：133例慢性荨麻疹患者随机分为中西医结合组(52例),中医组(38例)和西医组(43例)。中西医结合组口服消瘾方加减和依巴斯汀片10 mg/d治疗,治疗30天；中医组给予消瘾方加减治疗；西医组口服依巴斯汀片。根据患者瘙痒程度、风团大小、风团数目等评价疗效,ELISA检测3组患者IgE、IL－4及IFN－γ水平变化。结果：治疗第30天中西医结合组、中医组、西医组有效率分别为94.23％、71.05％和74.42％。随访6个月,中西医结合组、中医组、西医组复发率分别为18.37％、33.33％和53.13％。治疗后中西医结合组IgE和IL－4水平降低、IFN－γ水平升高较其它两组更为明显,具有统计学意义(均P<0.05)。结论：消瘾方加减联合依巴斯汀治疗慢性荨麻疹临床疗效好、复发率低,IgE、IL－4和IFN－γ水平变化对临床疗效及复发率的判断有一定的意义。%Objective: To assess the efficacy of Xiao-yin decoction combined with ebastine in the treat-ment of chronic urticaria ( CU) , and of the influence on the levels of IgE, IL-4 and IFN-γ. Methods:One hundred and thirty-three patients were randomly divided into Xiao-yin decoction combined with ebstine group (52 patients), Xiao-yin decoction group (38 patients) and ebastine group (43 patients). The efficacy was assessed according to the itching degree, numbers and sizes of wheals. The levels of IgE, IL-4 and IFN-γwere detected by ELSA. Results:The effective rates in the Xiao-yin decoction combined with ebstine group, Xiao-yin decoction group and ebastine group were 94.23%, 71.05% and 74.42% after 30 days of the treat-ment, respectively. The recurrence rates in the Xiao-yin decoction combined with ebstine group, Xiao-yin de-coction group and ebastine group were 18.37%, 33.33% and 53.13% after 6 months follow-up. The reduction in the levels of IgE and IL-4 and the increase in the level of IFN-γin the Xiao-yin decoction combined with ebstine group were more obvious than that in the other two groups, with significant differences (P<0.05). Conclusion: Xiao-yin decoction combined with ebastine in the treatment of CU is effective in the treatment of CU. The change in the levels of IgE, IL-4 and IFN- γ are helpful for estimating the prognosis.
