依托信息系统实现“五师”服务临床新模式

摘要

目的：实现“五师”服务临床的新模式，让医师、护师、药师、营养师、心理咨询师全面参与诊疗全过程。方法：一体化医护工作站以会诊申请的方式邀请药师、营养师和心理咨询师参与诊疗活动，收到会诊申请后，药师、营养师和心理咨询师通过信息系统开展“药学监护”、“营养筛查”和“心理评估”。结果：药师、营养师和心理咨询师通过信息系统与医师、护师进行灵活的信息交互，全面参与到诊疗全过程。结论：信息系统的有效使用，促进了医院“五师”服务临床全新理念的落实，为患者提供全方位、个性化的诊疗服务，进而提高患者信任度和忠诚度。%Objective: To establish the new mode of "Five Medical Staffs" clinical service by integrating clinical services by five types of medical staff—doctors, nurses, clinical pharmacists, dieticians, and psychological consultants. The "Five Medical Staffs" all participate in the whole process of diagnosis and treatment. Methods: Doctors request professional service from clinical pharmacists, dieticians and psychological consultants by the consultation application of EMRs. Clinical pharmacists, dieticians and psychological consultants will develop pharmaceutical care, nutritional screening and psychological assessment after they receive the requests from doctors. Results: The clinical information can be exchanged flexibly between the Five Medical Staffs by the usage of EMRs, and the Five Medical Staffs all participate in the whole process of diagnosis and treatment indeed. Conclusion: The effective usage of the information system promoted the implementation of the service mode of Five Medical Staffs clinical service, and provided full range and personalized clinical services to patients to increase patients' satisfaction and loyalty.
