目的 比较静脉化疗和动脉栓塞化疗两种途径新辅助化疗在IIB期宫颈癌中的近期疗效.方法 收集安徽省立医院2006年1月至2010年12月行VBP(长春新碱+博来霉素+顺铂)方案新辅助化疗的IIB期宫颈癌患者临床病例资料85例,其中静脉化疗(IVCT)44例,动脉栓塞化疗(TACE)41例,比较两种途径新辅助化疗的化疗有效率、化疗后手术率、各组不同直径肿瘤化疗疗效、手术时间、术中出血量、并发症及各种术后病理高危因素发生率.结果 两组化疗有效率、化疗后手术率比较有统计学意义(P= 0.03),各组不同直径肿瘤化疗疗效比较有统计学意义(P=0.009,0.013).手术时间、术中出血量和并发症比较均无统计学意义(P=0.233,0.767,0.845).术后病理高危因素如宫旁浸润及深肌层浸润比较有统计学意义(P=0.043,0.028),而淋巴结转移、脉管癌栓转移等比较无统计学意义(P=0.304,0.158).结论 IIB期宫颈癌中动脉栓塞化疗近期疗效优于静脉化疗;肿瘤直径大小影响新辅助化疗疗效.%Objective To compare recent treatment effects between intravenous chemotherapy and transarterial chemoembolization with cervical cancer in stage IIB. Methods We collected 85 patients' clinical data with cervical cancer in stage IIB admitted to Anhui Provincial Hospital with a VBP scheme of neoadjuvant chemotherapy from January 2006 to December 2010, which were assigned into 2 groups: intravenous chemotherapy(IVCT) group(44) .transarterial chemoembolization (TACE )group(41). Effective rate, surgical rate, ch.emotherEpeutic effect of different diameter tumor,surgery time,blood foss,surgical complications and the various pathological risk factors incidence were compared between the two groups. Results There were some significant differences in effective rate,surgical rate(P = 0. 03) . There were also some significant differences in the chemotherapeutic effect of different diameter tumor in the two groups ( P = 0.009,0.013). Surgery time,blood loss and surgical complications were not significantly different in the two groups(P = 0. 233,0.767 , 0.845) ;there were some differences in parametrial involvement and deep muscle infiltration(P = 0.043,0.028) ;lymph node metastasis and vascular carcinoma bolt transferring in the two groups were not significantly different(P =0. 304,0. 158) . Conclusion Transarterial chemoembolization is better than intravenous chemotherapy in recent effect. Tumor diameter influences the effect of neoadjuvant chemotherapy.
