卡介苗/结核菌纯蛋白衍化物对儿童哮喘临床控制及血清IL-4、IFN-γ、IL-10水平的影响

         

摘要

Objective To observe the effect of Bacillus Calmette-Guerin( BCG ) vaccine and purfied protein deriva-tives( PPD ) tuberculin on the control level and lung function of asthmatic children. To provide theoretical basis for the prevention and treatment of asthma. Methods Selected 68 cases was asthma children, which was from 5 to 14 years old, whose PPD test results were negative, divided randomly into BCG/PPD group and non-intervention group, 10 healthy children were taken as normal control group,before treatment ( 0 months ) the intervention group was vaccinated BCG, four weeks after inoculation with the PPD skin test to determine the succession,then they were given intradermal injection 5 units of PPD in the monthly, total six months; Before the start of treatment ( 0 months ) and after treatment ( 6 months ),respectively, IL-4,IFN-γ, IL-10 levels were detected and Clinic control level,without the intervention group were given routine treatment only and the observed indicators same as the intervention group. Results CD After 6 months of treatment, compared with the intervention group, respiratory tract infections and the times mitigation drugs had significant difference( P < 0. 01 ) in non-intervention group, need to see the e-mergency and times of PEF < 80% predicted a significant difference( P < 0. 05 ). (2) Before the start of treatment ( 0 months ), compared with the intervention group, IFN-γ, IL-4, IL-10 and IFN-γ/IL-4 levels were no significantly difference in non-intervention group, but IL-4 level in the two groups was higher than that in normal healthy children, IFN-γ, IL-10 and IFN-γ/IL-4 levels were lower than normal healthy children,the difference was statistically significant ( P <0. 01 ). After the end of treatment ( 6 months ), compared with pre-treatment, T cells factor IL-4, IFN-γ, IFN-γ, IL-10 and IFN-γ/IL-4 levels of the intervention group increased, IL-4 levels decreased, the difference was statistically significant ( P <0. 01 ). Compared with pre-treatment increased,IFN-γ,IL-10 and IFN-γ/IL-4 levels of the non-intervention group, IL-4 levels decreased, the difference was statistically significant ( P < 0. 05 ). Conclusion BCG/PPD intervention can improve the clinical efficacy and the immune disordered function state of asthmatic children.%目的 以卡介苗/结核菌纯蛋白衍化物(BCG/PPD)作为干预因素,观察干预因素对哮喘儿童的临床疗效及对白介素-4(IL-4)、γ-干扰素(IFN-γ)、白介素-10(IL-10)水平的影响,为BCG/PPD在支气管哮喘中的防治提供根据.方法 使用PPD试验筛选出68例5~14周岁PPD试验阴性的哮喘患儿,并随机分为BCG/PPD干预组(38例)和非干预组(30例),健康儿童为正常对照组(10例).治疗前(0月)干预组在常规治疗基础上接种BCG,4周后用PPD皮试判定接种成功后,每月皮内注射PPD 5单位,共6个月;治疗开始前(0月)和疗程结束后(6月),评估哮喘临床控制情况并采血检测IL-4、IFN-γ、IL-10水平;未干预组仅给予常规治疗,观察指标同干预组.结果 ① 治疗6个月后干预组与未干预组比较:平均呼吸道感染、需要缓解药物次数,需看急诊及PEF<80%预计值次数差异有显著性(P<0.01,P<0.05).② 治疗前干预组、未干预组在血清IFN-γ、IL-4、IL-10 和IFN-γ/IL-4比值差异无显著性;干预组、未干预组IL-4、IFN-γ、IL-10和IFN-γ/IL-4比值差异有显著性(P<0.05);治疗后干预组、未干预组血清IFN-γ、IL-10和IFN-γ/IL-4较治疗前上升、IL-4水平下降差异有显著性(P<0.05),干预组与未干预组比较差异有显著性(P<0.05).结论 BCG接种PPD刺激可以提高儿童哮喘临床疗效,并可改善哮喘儿童的细胞免疫功能紊乱状态.
