目的 探讨基质金属蛋白酶-2(MMP-2)在成人腹股沟疝患者腹横筋膜和腹外斜肌腱膜组织中的表达,及外周血清中的含量变化与腹横筋膜和腹外斜肌腱膜组织中Ⅰ和 Ⅲ型胶原含量及其比值相关性.方法 切取40例开放式手术患者腹横筋膜组织和腹外斜肌腱膜组织,其中腹股沟斜疝25例,腹股沟直疝15例,无疝患者10例,同时分别抽取其外周血.采用免疫组化、ELISA方法分析MMP-2组织中的表达和血清中的含量、组织中Ⅰ型胶原含量;放射免疫法检测组织中Ⅲ型胶原含量.结果 对照组MMP-2弱表达,斜疝组MMP-2阳性表达,直疝组表达最强;血清MMP-2两两比较,差异均有显著性(P<0.05);与对照组相比,直疝组组织中Ⅰ型胶原含量和Ⅰ/ Ⅲ型胶原比值下降,差异均有显著性(P<0.05);与对照组相比,直疝组Ⅲ型胶原含量增加,差异有显著性(P<0.05).结论 MMP-2 对腹横筋膜、腹外斜肌腱膜组织胶原的降解作用与成人腹股沟疝尤其是直疝的形成有相关性.%To explore the relation of the expression of the matrix metalloproteinase-2 ( MMP-2 ) in hernia transverse fascia and external oblique fascia and the level of serum of type I , Ⅲ collagen of these tissues. Methods Total 50 tissue specimens of hernia transverse fascia and external oblique fascia during abdominal operation in patients including 25 cases indirect inguinal hernia, 15 cases direct inguinal hernia, 10 cases without hernia. At the same time the blood was extracted, the expression of MMP-2 were detected with immunohistochemitry. ELISA analyzed the level of MMP-2 in serum and type I collagen in tissue; RIA analyzed the level of type Ⅲ collagen in tissue. Results The expression of MMP-2 was weak in the control group, the group of the indirect inguinal hernia expressed positive, the group of direct hernia showed the strongest expression; The MMP-2 concentration in blood were significantly different pairwise comparisons ( P < 0. 05 ); The mean type I collagen and the ratio of type I / Ⅲ collagen of the direct inguinal hernia group were lower than those in the control group( P < 0. 05 ); The mean type Ⅲ collagen of the direct inguinal hernia group was higher than that in the control group( P < 0. 05 ). Conclusion There was correlations between MMP-2 and the degradation of collagen in the transverse fascia, external oblique fascia and the formation of adult inguinal hernia, especially of inguinal direct hernia.
