目的 探讨外周T细胞淋巴瘤(peripheral T-cell lymphoma,PTCL)患者骨髓中独立生长因子(growth factor independence 1,Gfi1)的表达及其与临床预后的关系.方法 选择确诊的63例PTCL患者为研究对象,其中外周T细胞淋巴瘤-非特异型(peripheral T-cell lymphoma,not otherwise specified,PTCL-NOS)29例,血管免疫母细胞性T细胞淋巴瘤5例,肠病型T细胞淋巴瘤8例,鼻NK/T细胞肿瘤17例,肝T细胞淋巴瘤4例.对照组是10例反应性增生性淋巴结炎(reactive hyperplastic lymphadenitis,RHL).应用RT-PCR法扩增Gfi1引物,并进行琼脂糖凝胶电泳;应用组织形态学观察及免疫组织化学法(SP法)检测PTCL的淋巴瘤组织和作为对照组的RHL患者淋巴组织中Gfi1的表达情况.采用Kaplan-Meier方法对生存资料进行分析,用log-rank方法进行检验.结果 RT-PCR结果显示,PTCL患者的淋巴瘤组织中Gfi1表达呈阳性,RHL患者的淋巴组织中Gfi1表达为阴性.63例PTCL患者中只有42例保存有石蜡块标本,31例检出Gfi1蛋白阳性表达(73.81 %,31/42),而Gfi1在对照组RHL中表达均为阴性.生存分析显示Gfi1是与患者生存期及预后相关的因素,其阳性表达组的预后明显差于阴性表达组(P＜0.05).结论 Gfi1是判断PTCL预后有价值的分子生物学指标.%Objective To investigate the expression of Gfil in patients with peripheral T-cell lymphoma(PTCL)and its relationship with clinical prognosis. Methods Sixty-three PTCL patients(who had been diagnosed in Tongji Hospital) and 10 patients with reactive hyperplastic lymphadenitis(RHL) were included in our study. The patients included 29 cases of PTCL, not otherwise specified(PTCL-NOS) ,5 cases of angioimmunoblastic T-cell lymphoma, 8 cases of enteropathy-type T cell lymphoma,17 cases of nasal NK/T cell lymphoma,4 cases of primary liver T-cell lymphoma. Gfil was detected by reverse transcription polymerase chain reaction(RT-PCR). Fortytwo cases of PTCL were examined for Gfil proteins by using immunohistochemistry. To explore the prognostic factors,survival analysis and log-rank test were done. All of the patients were followed up to December 2010. All the statistica! variance was carried out by SPSS 12. 0. Results RT-PCR revealed that there was positive Gfil expression in PTCL,but negative in RHL. The positive rate of Gfil protein expression in PTCL was 73. 81 ％. Relatively, Gfil protein expression was negative in RHL. The prognosis of Gfil positive expression group was worse than that of negative expression group(P＜0. 05). Conclusion Gfil can be used as an valuable prognostic factor to judge the biological behavior of PTCL.
