特发性高钙尿肾结石患者肾乳头组织中Runx2、Osterix的表达

         

摘要

目的 研究成骨样细胞转录因子Runx2、Osterix在特发性高钙尿(idiopathic hypercalciuria,IH)肾结石患者肾乳头组织中的表达,以及IH患者结石形成的发病机制.方法 筛选华中科技大学同济医学院附属同济医院IH肾结石患者8例(IH组),排除各种已知影响血清钙或者尿钙的继发疾病;另选取同期因肾肿瘤或非结石所致的无功能肾需要行肾切除术的患者8例作为对照(NC组).取16例患者肾乳头组织标本若干,采用实时荧光定量PCR和Western blot检测Runx2和Osterix mRNA以及蛋白的表达水平.结果 IH组Runx2 mRNA表达量为(1.437±0.115),而NC组Runx2 mRNA表达量为(1.037 ±0.027),两组间差异有统计学意义(P<0.05);IH组与NC组Osterix mRNA的表达量分别为(1.826±0.462)和(1.030±0.072),差异有统计学意义(P<0.05).Western blot检测Runx2蛋白在NC组和IH组表达分别为(1.585±0.389)和(1.642±0.395),差异无统计学意义.Osterix蛋白在NC组和IH组表达量分别为(1.138 ±0.143)和(1.621 ±0.164),差异有统计学意义(P<0.05).结论 IH肾结石患者肾乳头Runx2和Osterix mRNA表达增强为间质异位钙化特征,结石形成的病理过程可能与正常骨发生机制生理过程类似.%Objective To investigate the expression of osteoblast-like cell transcription factors Runx2 and Osterix in patients with idiopathic hypercalciuria(IH) and renal calculi, and the role of Runx2 and Osterix in the stone formation in IH patients. Methods Kight patients with IH and renal calculi(IH group) were enrolled. Secondary diseases that could affect serum calcium or urinary calcium were not found in these patients. Kight patients who underwent nephrectomy due to renal carcinoma or non-functioning kidney served as control. The renal papillary tissues of the 16 patients were sampled. Realtime quantitative PCR and Western blot were used to detect the protein and gene expression levels of Runx2 and Osterix. Results The mRNA expression level of Runx2 in IH group was (1. 437 + 0. 115) ,significantly higher than that in control group with the value being (1. 037 + 0. 027)(P<0. 05). The mRNA levels of Osterix in IH group and NC group were(1. 826 + 0. 462) , (1. 030 + 0. 072) , respectively,and there was significant difference(P<0. 05). The expression level of Osterix protein were (1. 138 + 0. 143) and (1. 621 + 0. 164) in the NC group and IH group respectively with significant difference found between the two groups(P< 0. 05). The expression level of Runx2 protein wereCl. 585 + 0. 389) and (1. 642 + 0. 395) in the NC group and the IH group respectively and showed no significant difference between the two groups(P>0. 05). Conclusion Knhanced expression of Runx2 and Osterix mRNA in the renal papillary tissues of patients with IH and kidney stones may be related to interstitial ectopic calcification. The mechanism of stone formation in IH patients may be similar to that of normal bone formation.
