The informatization monitoring and evaluation index system of the demonstration community of sports-medicine integration is an important foundation for standardizing the construction of demonstration communities, accelerating the pace of sports-medicine integration, and achieving the ultimate development goal. Using literature data method, interview investigation method, mathematical statistics method, the establishment of an informatization monitoring and evaluation index system in the demonstration community of sports and medicine integration was studied. The Delphi method was used to make the preliminary selected index system into an expert questionnaire. Indexes are screened and scored according to the importance of each index, and the average, standard deviation, and coefficient of variation of each index are calculated to determine the index system. Subsequently, the index weights were determined using the analytic hierarchy process (AHP) method, and 15 experts were issued a weighted comparison matrix questionnaire. The analytic hierarchy process was used to calculate the largest eigenvalues and corresponding eigenvectors of each judgment matrix, and tested the consistency of the judgment matrix. The research results show that the demonstration community of sports and medicine integration is led by the government, the neighborhood community provides site resources, and the community health service center and universities form a team of experts to build a four-in-one model of “government department-hospital-community-scientific research institute”. It is a model for the integration of sports and medicine to try and explore first. On this basis, an informatization monitoring and evaluation system consisting of 4 first-level indicators, 11 second-level indicators, and 42 third-level indicators is constructed.
