To investigate the potential role of apoptosis in the development of acute lung injury (ALI) and the interventional effect of Chinese herb 912 liquor. Methods: Forty ― six Wistar rats were randomly divided into three groups. In ALI group, 30 rats were intravenously infused with standard E. coli to reproduce the ALI model, and the samples were taken at 4,8 and 12 hours after bacterial challenge respectively. In Sham operation (SO) group, 8 rats were intravenously infused with normal saline in equal volume and the samples were collected at 12 hours. In 912 group, 8 rats were intravenously infused with Chinese herb 912 liquor half an hour after standard E. coli injection and the samples were obtained at 12 hours. The parameters of ALI including blood gas, lung index(LI),lung permeability index(LPI), tumor necrosis factor- α(TNF-α) and pulmonary pathology were measured. Apoptosis was detected by transmission electron microscopy, flow cytometry and TdT-mediated dUTP nick end labeling(TUNEL), respectively. Results: It was observed that alveolar spaces were narrowed and neutrophils exuded at 4 hours after E. coli challenge . With prolongation of the time the pulmonary damage was deteriorated; P_aO_2 decreased, LI, LPI and TNF-α increased, and in comparison with SO group, the differences were slgnificant (all P<0.01). TUNEL results shewed that in MLI group, parts of cells were positive and apoptotic cells were (71.15+-1.26)%, (12.39+-1.67)%, (6.72+-1.11)% at 4,8 and 12 hours, respectively, as detected by flow cytometry(all P<0.01). The results at first were increased and then declined, in comparison with SO group, all P<0.01. In 912 group the pulmonary function parameters above - mentioned were improved and apoptotic cells were increased up to (24.67 +-2.85)%, in comparison with ALI group, P<0.01. Conclusions: Apoptosis may play an important roles in the pathogenesis of ALI. In the early phase of ALI apoptosis is at the low level. Apoptosis, as a pathogenetic mechanism, accelerates the inflammatory responses of ALI modulated by TNF- αIn resolution phase (912 group) the pulmonary function is improved. Apoptosis, as a protective factor, may take part in the remodeling of the lung tissue.
